Comparison of fracture risk assessment tools in older men without prior hip or spine fracture: the MrOS study

Purpose We compared the performance of fracture risk assessment tools in older men, accounting for competing risks including mortality. Methods A comparative ROC curve analysis assessed the ability of the QFracture, FRAX® and Garvan fracture risk tools, and femoral neck bone mineral density (BMD) T-score with or without age to identify incident fracture in community-dwelling men aged 65 years or older (N=4994) without hip or clinical vertebral fracture or antifracture treatment at baseline. Results Among risk tools calculated with BMD, the discriminative ability to identify men with incident hip fracture was similar for FRAX (AUC 0.77, 95% CI 0.73, 0.81), the Garvan tool (AUC 0.78, 95% CI 0.74, 0.82), age plus femoral neck BMD T-score (AUC 0.79, 95% CI 0.75, 0.83) and femoral neck BMD T-score alone (AUC 0.76, 95% CI 0.72, 0.81). Among risk tools calculated without BMD, the discriminative ability to identify hip fracture was similar for QFracture (AUC 0.69, 95% CI 0.66, 0.73), FRAX (AUC 0.70, 95% CI 0.66, 0.73) and the Garvan tool (AUC 0.71, 95% CI 0.67, 0.74). Correlated ROC curve analyses revealed better diagnostic accuracy for risk scores calculated with BMD compared to QFracture (p < 0.0001). Calibration was good for the internally generated BMD T-score predictor with or without age and poor for the externally developed risk tools. Conclusion In untreated older men without fragility fractures at baseline, an age plus femoral neck BMD T-score classifier identified men with incident hip fracture as accurately as more complicated fracture risk scores.

Osteoporosis is the most common chronic metabolic bone disease. It has been estimated that more than 10 million people in the United States and 200 million men and women worldwide have osteoporosis. Given that the aging population is rapidly increasing in many countries, osteoporosis could become a global challenge with an impact on the quality of life of the affected individuals. Osteoporosis can be defined as a condition characterized by low bone density and increased risk of fractures due to the deterioration of the bone architecture. Thus, the major goal of treatment is to reduce the risk for fractures. There are several treatment options, mostly medications that can control disease progression in risk groups, such as postmenopausal women and elderly men. Recent studies on the basic molecular mechanisms and clinical implications of osteoporosis have identified novel therapeutic targets. Emerging therapies targeting novel disease mechanisms could provide powerful approaches for osteoporosis management in the future. Here, we review the etiology of osteoporosis and the molecular mechanism of bone remodeling, present current pharmacological options, and discuss emerging therapies targeting novel mechanisms, investigational treatments, and new promising therapeutic approaches.

Приводится обзор современного состояния проблемы диагностики и лечения остеопороза. У мужчин остеопороз представляет собой серьезную и недооцененную проблему. Каждый пятый мужчина в возрасте старше 50 лет в течение оставшейся жизни перенесет хотя бы один остеопорозный перелом. Ведущими факторами развития остеопороза у мужчин являются генетическая предрасположенность, снижение уровня половых гормонов и болезни, вызывающие вторичный остеопороз, который, по некоторым данным, может занимать до 40 % всех случаев заболевания у мужчин. Причинами вторич-ного остеопороза у мужчин чаще всего являются злоупотребление алкоголем, гиперкортицизм (эндо-генный или, чаще, экзогенный при приеме глюкокортикоидов) и гипогонадизм. Коррекция основного заболевания может привести к повышению минеральной плотности костной ткани (МПК) и, возможно, к снижению риска переломов. Для лечения остеопороза у мужчин зарегистрированы бисфосфонаты (алендронат, ризедронат и золедроновая кислота), деносумаб и терипаратид. Следует также не забывать о важности немедикаментозных методов лечения остеопороза, таких как отказ от вредных привычек, правильное питание и выполнение физических упражнений, направленных на снижение риска падений и переломов.Ключевые слова: остеопороз, мужской пол, диагностика, лечение. Mechnikov, St Petersburg, Russia; Ural State Medical University, Ekaterinburg, RussiaThe author revises the latest evidence in the literature regarding managing of osteoporosis in men. Osteoporosis is an important under-recognized problem threatening men. Among people over 50 years of age, a fifth of men will suffer a fragility fracture during their remaining lifetime. The leading risk factors of male osteoporosis are genetic predisposition, low sex hormones and secondary osteoporosis. The latter comprises up to 40% of all male osteoporosis cases. The most common causes of secondary osteoporosis in men are alcohol abuse, endogenic and exogenic hypercortisolism and hypogonadism. Treatment of the underlying disease may result in BMD increase and probably fracture risk improvement. Bisphosphonates (alendronate, risedronate and zoledronic acid), denosumab and teriparatide are approved for treatment of male osteoporosis. Non-pharmacological treatment including avoiding smoking and excessive drinking, health lifestyle and a diet rich in bone-healthy nutrients should also be considered.
