Comparative Effects of Nesiritide and Nitroglycerin on Renal Function, and Incidence of Renal Injury by Traditional and RIFLE Criteria in Acute Heart Failure

Abstract: The incidence of renal injury was not different between nesiritide- and nitroglycerin-treated patients with acute heart failure; however, nitroglycerin was associated with a decline in glomerular filtration rate and increase in blood urea nitrogen despite higher baseline and on treatment blood pressures.

“…The renal actions of M‐ANP were significantly enhanced compared to NTG and occurred despite a significant reduction in BP. NTG did decrease RVR and this response is consistent with the elegant work of Elkayam who has demonstrated the potent renal vasodilating action of NTG in human HF . Unlike M‐ANP, the renovascular dilatation with NTG is unassociated with an increase in natriuresis, diuresis, or GFR .…”

“…Tests of BUN and blood Cr are typically used to assess renal function 26. Scald injury led to about a twofold increase in both BUN and blood Cr ( P < 0.05 relative to the sham group; Figs.…”

“…We read with interest the recent article by Ng et al that described no difference in the incidence of acute renal injury in patients with acute decompensated heart failure (HF) receiving either nitroglycerin or nesiritide. 1 On the other hand, their results suggested preservation of renal function by nesiritide. This prompted us to report our findings on the effect of periodic outpatient administration of nesiritide on renal function in patients with refractory HF unresponsive to optimal oral therapy.…”
