The high incidence rate and wide range of musculoskeletal pathologies determine the improvement of the diagnostic process. Late diagnosis leads to complications, which in turn increase the percentage of disability. Therefore, the search for the most informative method with the least radiation load on the patient remains an urgent problem for radiologists. Cone beam computed tomography (CBCT) is a modern and promising technique that has already found wide application in dentistry and otorhinolaryngology. Among the advantages of CBCT are: three-dimensional image; high spatial resolution; low radiation dose. Thanks to technical improvements in equipment and the introduction of new image processing protocols, it has become possible to expand the indications for conducting the researches, including the researches based on imaging of the upper and lower extremities. Based on the results of a CBCT study, we can evaluate: the shape and contour of the bone; solution of continuity of the bone and malposition of bone fragments; the structure of bone tissue and the pathological processes occurring in it (destruction, osteoporosis, osteosclerosis); joint congruence and changes in articular surfaces surrounding soft tissues. Therefore, CBCT can be introduced into the diagnostic process of bones and joints diseases. The use of this technique will find wide application in traumatology and orthopedics (fractures, dislocations, post-traumatic deformities, aseptic necrosis, osteoarthritis), rheumatology (rheumatoid arthritis, polyarthropathy, juvenile arthritis, gout), surgery (osteomyelitis), oncology (benign and malignant bone tumors) both in the adult population and in pediatric practice. This paper presents a review of the literature, which examines the degree of development of the issue of using CBCT and describes study protocols and protocols for processing the obtained images in the diagnosis of musculoskeletal diseases.

Objectives: This research is aimed to compare the accuracy of periapical radiography in detecting periapical radiolucency with that of Cone-beam Computed Tomography (CBCT) and to assess the additional information that CBCT provides. Materials and Methods: 96 patients with a primary diagnosis of endodontic problem had been studied retrospectively. Each root was examined for the presence or absence of periapical lesions according to the Periapical Index (PAI) Score. Roots and root canals identified through perapical radiography and CBCT were recorded. Additional information from CBCT regarding effects of lesions in cortical bone and maxillary sinus was also noted. Sensitivity, specificity, positive predictive value, and negative predictive value were analyzed. Results: The result showed that accuracy of periapical radiography as expressed by its sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV) was 0.65, 0.90, 0.86, 0.75, respectively. Conclusion: Periapical radiography has a low sensitivity in detecting periapical lesions compared to CBCT. CBCT also provides more detailed information that is useful in endodontic treatment.

ObjectivesDiagnostic imaging is crucial for implant dentistry. This review provides an up‐to‐date perspective on the application of digital diagnostic imaging in implant dentistry.MethodsElectronic searches were conducted in PubMed focusing on the question ‘when (and why) do we need diagnostic imaging in implant dentistry?’ The search results were summarised to identify different applications of digital diagnostic imaging in implant dentistry.ResultsThe most used imaging modalities in implant dentistry include intraoral periapical radiographs, panoramic views and cone beam computed tomography (CBCT). These are dependent on acquisition standardisation to optimise image quality. Particularly for CBCT, other technical parameters (i.e., tube current, tube voltage, field‐of‐view, voxel size) are relevant minimising the occurrence of artefacts. There is a growing interest in digital workflows, integrating diagnostic imaging and automation. Artificial intelligence (AI) has been incorporated into these workflows and is expected to play a significant role in the future of implant dentistry. Preliminary evidence supports the use of ionising‐radiation‐free imaging modalities (e.g., MRI and ultrasound) that can add value in terms of soft tissue visualisation.ConclusionsDigital diagnostic imaging is the sine qua non in implant dentistry. Image acquisition protocols must be tailored to the patient's needs and clinical indication, considering the trade‐off between radiation exposure and needed information. growing evidence supporting the benefits of digital workflows, from planning to execution, and the future of implant dentistry will likely involve a synergy between human expertise and AI‐driven intelligence. Transiting into ionising‐radiation‐free imaging modalities is feasible, but these must be further developed before clinical implementation.
