Laser speckle contrast imaging (LSCI) enables continuous high-resolution assessment of microcirculation in real-time. We applied an endoscope to LSCI to measure cochlear blood-flow in an ischemia–reperfusion mouse model. We also explored whether using xenon light in combination with LSCI facilitates visualization of anatomical position. Based on a previous preliminary study, the appropriate wavelength for penetrating the thin bony cochlea was 830 nm. A 2.7-mm-diameter endoscope was used, as appropriate for the size of the mouse cochlea. Our endoscopic LSCI system was used to illuminate the right cochlea after dissection of the mouse. We observed changes in the speckle signals when we applied the endoscopic LSCI system to the ischemia-reperfusion mouse model. The anatomical structure of the mouse cochlea and surrounding structures were clearly visible using the xenon light. The speckle signal of the cochlea was scattered, with an intensity that varied between that of the stapes (with the lowest signal), the negative control, and the stapedial artery (with the highest signal), the positive control. In the cochlear ischemia–reperfusion mouse model, the speckle signal of the cochlea decreased during the ischemic phase, and increased during the reperfusion phase, clearly reflecting cochlear blood-flow. The endoscopic LSCI system generates high-resolution images in real-time, allowing visualization of blood-flow and its changes in the mouse cochlea. Anatomical structures were clearly matched using LSCI along with visible light.

Introducción La hipoacusia neurosensorial súbita es una emergencia otorrinolaringológica descrita desde 1944 y que continúa siendo un gran enigma. El objetivo es resumir e identificar los aspectos relevantes, avances y futuras perspectivas acerca del tema Metodología: Se realizó una revisión de literatura usando los motores de búsqueda de MEDLINE y Google académico de los últimos 5 años. Resultados: Es de causa idiopática en el 85% a 90%, con presentación bilateral en 0,4 a 3,4%, se han planteado numerosas teorías etiológicas entre ellas: vascular, inmunológica, estrés oxidativo, autoinmune, fístula perilinfática, traumáticas, viral, tumoral,entre otros, y en relación a ellas, numerosas líneas de tratamiento sin evidencia concluyente hasta el momento. Discusión: Frente a este cuadro clínico, se debe actuar inmediatamente; en el diagnóstico es importante la diferenciación con hipoacusia secundaria, la detección de signos de alarma, la forma de presentación y la severidad de la hipoacusia. No existe al momento un consenso global sobre su manejo, sin embargo, se recomienda iniciar con corticoterapia de manera inmediata, El uso de otras terapias aún se encuentra en debate, parecería tener evidencia prometedora el uso adyuvante con corticoides de vasodilatadores (PGE1), calcio antagonistas y acupuntura. La terapia de rescate parecería ser prometedora con el uso de oxígeno hiperbárico y corticoides intratimpánicos. Se han planteado otras terapias a futuro, buscando la estimulación de la regeneración de cilios celulares, o el reemplazo con células madre, sin embargo, estas terapias son conceptuales y aun no se han comprobado en estudios experimentales.

Background：The irrational use of medicines has become a major problem worldwide. The World Health Organization (WHO) estimates that more than half of all medicines are prescribed, dispensed, or sold inappropriately, and advised the pursuit of the responsible use of medicines.In China regulations require that a pharmacist must audit the prescription.Alprostadil injection is a vasodilator and an inhibitor of platelet aggregation There is no obvious or clear indication for its use in the ear, nose and throat (ENT) Department. It came to our notice that in our hospital, alprostadil injection has been prescribed in the ENT Outpatient Department to treat some diseases. Aim: This utilization review on the use of alprostadil injection was carried out in the ear, nose and throat (ENT) Outpatient Department of our hospital to determine its utilization patterns and promote its rational use. Methods: A prescription survey was conducted, and detailed demographic information, including the patient’s age, gender, diagnosis and so on, were recorded in an Excel spreadsheet. The rationality for these prescriptions were retrospectively evaluated.Results: Based on the evaluation results, all alprostadil injections were found to be used off label in the Otolaryngology Department. Furthermore, 98.3% of the prescriptions were for the treatment of sudden hearing loss, 1.1% of the prescriptions for the diagnosis of hypoacusis were considered empirical and should have been more specific, and 0.6% of the prescriptions were for the treatment of Ménière’s disease and large vestibular aqueduct syndrome. There were various dosage regimens for alprostadil injection in clinical practice. However, there was no statistically significant difference in dosage between the different age groups. Moreover, most of the alprostadil injections were given by IV bolus, while 12.2% of patients received this by intravenous drip.Conclusion: The present review indicated that improvements should be made in the quality of prescriptions for alprostadil injection in the Otolaryngology Outpatient Department. Furthermore, there is no strong evidence to support the use of high doses. Future audits of such use may be combined with an awareness campaign to reduce and discourage the irrational use of off label medications.
