(1) Background: The use of navigation systems is rarely necessary for routine sinus surgery. They may prove to be advantageous for difficult operations, for example, in finding structures that are difficult to reach, in the treatment of cancers, or in revisional surgery. Navigation systems are also said to have positive effects on the self-confidence of surgeons in stressful situations and in the training of doctors. (2) Methods: This retrospective study included patients who underwent surgical treatment for chronic sinusitis from 2012 to 2016 at the ENT clinic of the University Hospital, Erlangen. Two groups were formed; one includes patients without navigated sinus surgery, the other includes those with navigation. The incision–suture times of both groups and cost analysis are compared. An appropriate cost estimate for sinus surgery is determined. (3) Results: From the available results, no economically efficient navigation systems in sinus surgery at the ENT clinic can be shown. The main reason is that lengthening the operating time leads to higher costs. (4) Conclusions: Although the use of a navigation system for endonasal sinus surgery cannot be economically justified, it is an important tool, especially in cases with complex anatomical conditions, and the system is essential for training purposes.

Endoscopic sinus surgery(ESS)is one of the most common surgeries performed for chronic sinusitis, but it carries a high risk of damaging adjacent organs, such as the skull base and orbit 1) . To minimize this risk, ESS is often performed utilizing an optical or magnetic surgical navigation system. However, there are still many reports of complications included CSF leak, diplopia, postoperative hemorrhage 2) ,3) . Skill learning support is necessary for the improvement of surgical outcomes. In recent years, there are many studies have been reported on ESS education methods using a sim-ulator based on real anatomical models or Virtual Reality (VR) . Abdulaziz et al. developed a 3D-printed model based on CT images of the patient for the training of ESS skills 4) . Also, there are many VR simulators with haptic feedback 5) ,6) . Samuel et al. developed a comparatively lowcost mixed-reality simulator combining VR and 3D-printed models 7) . In addition to the fact that the simulator can practice repeatedly, there are also a report that the experience level of the surgeon and the result of the simulator have a positive correlation 6) . Therefore, it can be said that simulator training is a useful training tool for resident. However, it is pointed out that there are few products that currently have been commercialized and they are very expensive, and there is no platform that can be widely used in training programs 8) -10) . I. Executive Summary Background The 5 years since the publication of the first International Consensus Statement on Allergy and Rhinology: Rhinosinusitis (ICAR‐RS) has witnessed foundational progress in our understanding and treatment of rhinologic disease. These advances are reflected within the more than 40 new topics covered within the ICAR‐RS‐2021 as well as updates to the original 140 topics. This executive summary consolidates the evidence‐based findings of the document. Methods ICAR‐RS presents over 180 topics in the forms of evidence‐based reviews with recommendations (EBRRs), evidence‐based reviews, and literature reviews. The highest grade structured recommendations of the EBRR sections are summarized in this executive summary. Results ICAR‐RS‐2021 covers 22 topics regarding the medical management of RS, which are grade A/B and are presented in the executive summary. Additionally, 4 topics regarding the surgical management of RS are grade A/B and are presented in the executive summary. Finally, a comprehensive evidence‐based management algorithm is provided. Conclusion This ICAR‐RS‐2021 executive summary provides a compilation of the evidence‐based recommendations for medical and surgical treatment of the most common forms of RS.

Abstract: We developed a method to enable a time-series comparative analysis of surgical processes based on the surgical data from the navigation system. This method can allow surgeons to identify differences between their procedures and reference procedures such as experts' procedures.
