Comparative analysis of density histograms and visual scores in incremental and volumetric high-resolution computed tomography of the chest in idiopathic pulmonary fibrosis patients

Abstract: Background Volumetric high-resolution computed tomography (HRCT) of the chest has recently replaced incremental CT in the diagnostic workup of idiopathic pulmonary fibrosis (IPF). Concomitantly, visual and quantitative scores have been proposed for disease extent assessment to ameliorate disease management. Purpose To compare the performance of density histograms (mean lung attenuation, skewness, and kurtosis) and visual scores, along with lung function correlations, in IPF patients submitted to incremental o… Show more

“…Acute exacerbation of ILD is an acute clinical deterioration in patient with a known (or not) ILD, without a clear reason demonstrable at HRCT such as pulmonary embolism, edema, infection, or left heart failure [ 61 63 ]. Idiopathic pulmonary fibrosis (IPF) and other ILD can cause these HRCT patterns characterized by diffuse GGO in both lungs on a background of pre-existing areas of pulmonary fibrosis (Fig.…”

“…Consequently, alternative objective quantitative methods have been actively explored. Volumetric quantitative CT have been used to predict lung fibrosis outcome 6 8 for patient stratification or prognosis in COPD, systemic sclerosis, early chronic lung allograft dysfunction in lung transplant patients 9 , ARDS, and recently in COVID-19 patients 10 13 . Two main approaches coexist in Quantitative volumetric CT densitometry: (1) the (“first order”) radiomic method uses the whole lung CT density histogram to extract statistical features such as mean lung attenuation (MLA) standard deviation, skewness and kurtosis, quantile predictors (median, 75th percentile density, etc.)…”

“…Foram criadas ferramentas tanto de código aberto como comerciais para traçar o perfil do paciente e estratificar o prognóstico de maneira melhor, com dados matemáticos e estatísticos. (9,10) A inteligência artificial é uma possibilidade emergente aguardada com esperança. As DPI fibróticas representam um setor altamente complexo da medicina, que exige a integração de conhecimentos específicos e profundos, além de uma interação estreita entre profissionais diferentes (e complementares).…”
