Importance: Retinal ganglion cells endure significant metabolic stress in glaucoma but maintain capacity to recover function. Nicotinamide, a precursor of NAD + , is low in serum of glaucoma patients and its supplementation provides robust protection of retinal ganglion cells in preclinical models. However, the potential of nicotinamide in human glaucoma is unknown. Background: To examine the effects of nicotinamide on inner retinal function in glaucoma, in participants receiving concurrent glaucoma therapy. Design: Crossover, double-masked, randomized clinical trial. Participants recruited from two tertiary care centres. Participants: Fifty-seven participants, diagnosed and treated for glaucoma. Methods: Participants received oral placebo or nicotinamide and reviewed six-weekly. Participants commenced 6 weeks of 1.5 g/day then 6 weeks of 3.0 g/day followed by crossover without washout. Visual function measured using electroretinography and perimetry. Main outcome measures: Change in inner retinal function, determined by photopic negative response (PhNR) parameters: saturated PhNR amplitude (Vmax), ratio of PhNR/b-wave amplitude (Vmax ratio).

Objective: To compare the efficacy between Ahmed glaucoma valve surgery and Trabeculectomy with Mitomycin-c in pediatric glaucoma. Study design and setting: Quasi-experimental study was conducted at Pediatric Ophthalmology Department, ISRA Post graduate Institute of Ophthalmology/ Al-Ibrahim Eye Hospital, Karachi. Duration of study was February to August 2016. Methodology: Samples were divided into two groups. Group A Trabeculectomy with Mitomycin-C and Group B with Ahmed Glaucoma valve surgery. Inclusion criteria were children (above 2 years and less than 15 years of age) visiting Pediatric Ophthalmology OPD irrespective of gender, glaucoma diagnosed with the duration of symptoms > 4 weeks. Exclusion criteria were patient with repeat surgery for glaucoma, combined ocular surgery i.e penetrating keratoplasty or cataract, Preoperative keratitis, uveitis or conjunctivitis, as determined by slit-lamp examination. Paired sample t-test was applied to compare the pre and post IOP examination findings. P-value = 0.05 considered as statistically significant. Results: A total of 110 patients were recruited in this study in which 97 patients were analyzed. Mean age of Group A was 6.73±2.7 and mean age of Group B was 7.05±2.3. Mean pre-operative IOP in “Group A” was found to be 31.59±5.4 and post-operative IOP was reduced to 17.95±4.8 with statistically significant P-value <0.0001. In “Group B” Mean preoperative IOP was 30.60±5.3 and post-operative IOP was 18.43±3.8 with statistically significant P-value <0.0001. Conclusion: A significant difference found between efficacy of Ahmed glaucoma valve surgery and Trabeculectomy with Mitomycin-C in pediatric glaucoma after 4th week of surgery.

Background: Measurement of intraocular pressure (IOP) forms a crucial component in the diagnosis and management of glaucoma. Many devices have been developed to measure IOP with Goldmann applanation tonometry (GAT) considered the gold standard for IOP measurement. Objectives: The objective was to compare the IOP measured using Icare ic100 and GAT. Methods: This cross-sectional study measured IOP in 1,000 eyes (500 left and 500 right) using the Icare ic100 and GAT. Central corneal thickness (CCT) was measured using a hand-held pachymeter. IOP measurements were investigated in all eyes, by IOP substrata, by CCT group, by number of topical glaucoma medications, and diagnosis. Results: There was moderate agreement between ic100 and GAT IOP measurements (intraclass correlation coefficient 0.73). Mean IOP was significantly lower when measured by ic100 than by GAT (12.1 vs. 16.2 mm Hg, p < 0.0001). Mean ic100 IOPs were also significantly lower than mean GAT IOPs within each IOP strata ≤12 (7.9 vs. 9.7 mm Hg, p < 0.0001), 13–21 (12.1 vs. 16.6 mm Hg, p < 0.0001), and ≥22 (18.4 vs. 25.2 mm Hg, p < 0.0001) and within each subanalysis. Conclusions: The Icare tonometer consistently under estimated IOP compared to GAT, irrespective of CCT ranges and other subgroup analyses. The mean difference of 4.2 mm Hg can have significant clinical implications, particularly in the management of glaucoma patients.
