However, sustained engagement is a major barrier to collection of PGHD. Little is known about barriers to sustained engagement or strategies to intervene upon engagement through application design.Objective This article investigates individual patient differences in sustained engagement among individuals with a history of AF who are self-monitoring using mHealth technology. Methods This qualitative study involved patients, health care providers, and research coordinators previously involved in a randomized, controlled trial involving electrocardiogram (ECG) self-monitoring of AF. Patients were adults with a history of AF randomized to the intervention arm of this trial who self-monitored using ECG mHealth technology for 6 months. Semistructured interviews and focus groups were conducted separately with health care providers and research coordinators, engaged patients, and unengaged patients. A validated model of sustained engagement, an adapted unified theory of acceptance and use of technology (UTAUT), guided data collection, and analysis through directed content analysis.Results We interviewed 13 patients (7 engaged, 6 unengaged), 6 providers, and 2 research coordinators. In addition to finding differences between engaged and unengaged patients within each predictor in the adapted UTAUT model (perceived ease of use, perceived usefulness, facilitating conditions), four additional factors were identified as being related to sustained engagement in this population. These are: (1) internal motivation to manage health, (2) relationship with health care provider, (3) supportive environments, and (4) feedback and guidance. Conclusion Although it required some modification, the adapted UTAUT model was useful in understanding of the parameters of sustained engagement. The findings of this study provide initial requirement specifications for the design of applications that engage patients in this unique population of adults with AF.

Wireless Sensor Networks (WSNs) is a type of adhoc network that is use to sense some phenomena with the help of sensor nodes. The nodes have scarce resources like power, storage, processing power, sensing and communication. Now a day WSNs are helping as a main constituting component for variety of applications like intelligence gathering, battle field monitoring, pollution mapping, smart cities, smart homes and health care monitoring systems etc. To make such system a reality and work with reliability, data security needs a lot of attention to be handled properly. Due to scarce resources in WSNs, implementation of proper security technique is not quite simple rather it is a challenging task to accomplish. Huge number of security algorithms have been proposed for WSNs, but among them network wide master key based security systems are more appropriate due to less overhead in establishing a secure channel. In this survey different security solution such as LEAP, BROSK, Spins, C & R, Light Weight Authentication Systems and ECC are investigated to study the impact of physical attack on them.

This article provides a thorough review of 5G wireless electrocardiography (ECG) communication for actual cardiac signal using both technical and clinical features for the purpose of recommending real‐time observation. Major issue regarding this frame work is the distortion and smoothness of ECG signal for the cardiologist advice also it needs correct diagnosis at the proper time. Extensive 5G wireless simulated scenarios are used to supervise the compressed ECG signal on reception. An efficient ECG transmission protocol is a modern and consistent ECG transmission protocol which is used to retransmit the noise signals. Thus, we are filtering the ECG signals to improve the quality which may directly affects the medical diagnosis. Because, time and frequency indicators can be obtained by the wavelet analysis but majority of the error functions cannot be flexible to various signals because of the static transition curve of threshold. Hence, we proposed a new emperor penguin optimization (EPO) together with wavelet thresholding (EPOWT) method. Here, the EPOWT is estimated through the means of real‐time ECG records. The outcomes of filtering process shows that the proposed EPOWT approach can achieve smooth threshold transition and soft thresholding bring the wavelet coefficient. EPOEPO algorithm is used to improve its flexibility to different signals. The EPOWT can generate an essential noise‐free signal by balancing the smoothness and signal distortion filtering for transmission. The proposed method can attains 2.9 seconds delay during simulation. Thus, the 5G wireless channel parameter's functioning zone is simple to acquire.
