Common bile duct stones management: A network meta-analysis

Abstract: BACKGROUND:Timely management is critical for treating symptomatic common bile duct (CBD) stones; however, a single optimal management strategy has yet to be defined in the acute care setting. Consequently, this systematic review and network meta-analysis, comparing one-stage (CBD exploration or intraoperative endoscopic retrograde cholangiopancreatography [ERCP] with simultaneous cholecystectomy) and two-stage (precholecystectomy or postcholecystectomy ERCP) procedures, was undertaken with the main outcomes o… Show more

“…The management of choledocholithiasis has evolved over the past decade with a growing body of literature that supports single stage therapy (i.e., operative intervention with concomitant ductal clearance) for those with symptomatic disease. [15][16][17] This can be achieved through several intraoperative modalities that will be described in the following sections. Institutional expertise and availability will ultimately dictate which single stage care path is undertaken.…”

“…The recurrence after successful CBD stone clearance reaches up to 8.4% within a median time of 2.5 years, and it is more often found after ERCP-ES than after LCBDE[ 18 , 22 ]. This is particularly related to some morphological subtypes (S and polyline type) of CBD[ 23 ], and regular follow-up is necessary in cases with risk factors[ 24 ].…”

“…Elevated liver function tests have positive predictive value. In most patients, serum bilirubin, alkaline phosphatase, and gamma-glutamyltransferase are elevated, which has the highest sensitivity and specificity for the diagnosis of common bile duct stones [ 10 ].…”
