Commentary on A Neglected Acne Scar Type: Papular Acne Scars and Their Correlations With Keloid Scars

Abstract

A cne is a protean disease comprising open and closed comedones, papular and pustular diseases, and cystic and nodular lesions. 1,2 It is hardly a surprise that the scars that emanate from this inflammation and lesion resolution are also variable in type and severity.Acne is hereditary albeit common. The induction of this disorder involves alterations in behavior of the sebaceous gland, sebum and the follicular apparatus, and hormones and to some extent lifestyle, but the efficiency of the healing process seems to dictate the tendency for some people to scar and some with similar severity not to do so. 3 It would seem that a robust innate immune response delivers brisker acute inflammation that heals better than the slower more chronic inflammatory response of a muted innate immunity. 4,5 Diversified multiple genes are at play in the production of acne 6 and there is a shift in T-helper type 1 (Th1) and Th17 and an upregulation in TNF-a. It may be that the key to acne is to find more directed and effective treatments by targeting its immune aberrations.The author has indicated no significant interest with commercial supporters.
