Combined Use of High Radiofrequency Disk Ablation, Annulus Modulation, and Manual Nucleotomy in a Patient with Extruded Disk Herniation

Abstract: A 43-year-old male had magnetic resonance imaging (MRI) demonstrating an extruded disk herniation in the left foraminal region, compression at the left spinal nerve root, and obliteration of the left foraminal entrance of the L5-S1 distribution. In the operating room, sufficient disk material was removed using grasping forceps, and then, the Disc-FX system with a Trigger-Flex probe (Elliquence, Baldwin, NY, U.S.A.) was inserted. Modulation of the annulus was performed in bipolar hemo mode, and nucleus ablation… Show more

“…Kyung‐Chul Choi et al vaporized a down‐migrated L4–5 disc by epiduroscopic Ho:YAG laser application in 1 case and successfully removed the disc by the endoscopic method. Beyaz et al reported that they treated a patient with far‐lateral extruded disc herniation and a low foot with percutaneous discectomy and radiofrequency ablation. In this study, we treated patients with MSU 3AB herniated discs using a combined technique of percutaneous and epiduroscopic routes with a success rate of 82.9%.…”

“…Finally, if difficulty was encountered attempting to locate the inferomedial genicular nerve, the Triggerflex RFA probe was utilized in a 360-degree manner to offset this limitation. The Triggerflex RFA probe allows easy implementation through working channels for navigational, targeted application of RF treatment on target tissues and has been utilized in several notable endoscopic procedures (11,17).…”

“…Since patients with bony foraminal stenosis were excluded from the study, a routine foraminoplasty was not necessary. If bleeding occurred, a radiofrequency probe (Elliquence ® Baldwin, NY) was used for coagulation [ 46 ]. The endoscopic decompression procedure was directly visualized throughout the surgery.…”
