Combined Treatment of Photodynamic Therapy and Bevacizumab for Choroidal Neovascularization Secondary to Age-Related Macular Degeneration

Abstract: PurposeTo evaluate the outcome of a combined photodynamic therapy and intravitreal injection of bevacizumab in choroidal neovascularization secondary to age-related macular degeneration.MethodsPhotodynamic therapy (PDT) was administered to 28 eyes followed by 3 consecutive bevacizumab injections. Patients were followed-up for more than 12 months. At baseline, 1, 3, 6, and 12 months post PDT, visual acuity (VA) and central macular thickness were measured using optical coherence tomography.ResultsThe mean VA was… Show more

“…98,99 Usually, the cases with poorer prognosis and poorer response to treatment are those with severe structural abnormalities like the formation of large disciform scars and collateral damage to the RPE cells and the overlying retina, influenced by the activity of fibroblast and local inflammation. 99,100 Combination approaches may not only increase overall efficacy but also reduce the need for monthly injections and side effects by allowing the use of lower doses to yield greater levels of efficacy than higher doses of a single drug. The combined therapy approaches allows targeting more than one component of the disease process at a time.…”

“…Neovascular age-related macular degeneration (nAMD) is a major cause of vision loss among the elderly population in Western countries [ 1 2 ]. Before the era of anti-vascular endothelial growth factor treatment, visual loss prevention was only achieved in a limited number of patients despite the use of different treatment modalities [ 3 4 5 6 7 ]. Bevacizumab, ranibizumab, and finally aflibercept treatments have led to conservation of baseline visual acuity (VA) in the vast majority of patients and significantly increased VA in approximately one-third of patients [ 8 9 10 11 12 ].…”

“…This case series assessed the efficacy of combining full-fluence PDT with anti-VEGF therapy in patients with a PED from exudative AMD. This combined treatment has been attempted in those with CNV from AMD, with promising results with respect to visual acuity 21 and reduction of treatments. 22 However, this is the first study to assess this treatment for PEDs.…”
