Combined transanal and laparoscopic approach for the treatment of deep endometriosis infiltrating the rectum

Abstract: Despite the small numbers in this series, our data suggest that the new technique of transanal rectal disc excision using the contour stapler may be applied in patients with infiltrating endometrial nodules of the rectum up to 10 cm from the anal margin and up to 5 cm in diameter. This new procedure promises to be a useful addition to the surgeon's armamentarium in a multidisciplinary approach to deep pelvic endometriosis.

“…The occasional discovery of an asymptomatic DIE nodule should not always lead to surgery, only those lesions that give rise to symptoms should be surgically managed . Indeed, in agreement with other authors, we believe that the aim of surgical treatment should be the relief of pain as opposed to a compulsive resection of endometriotic lesions irrespective of the risks of post‐operative complications .…”

“…Depth of shaving is difficult to control in large infiltrations of the rectum (19), and the surgeon limits removing fibrous tissue up to the submucosa due to risk of postoperative fistula. Disc excision using the transanal end-to-end circular staplers is not feasible in large colorectal nodules, as the stapler cannot accommodate a volume of tissue exceeding 7 cmc (20,21). Disc excision using the transanal Contour Transtar stapler can be performed in large nodules of the lower and mid rectum, however their cranial limit must not be further than 8 cm from the anus (22).…”

“…The Contour ® Transtar ® ‐STR5G (Ethicon EndoSurgery Inc., Cincinnati, Ohio, USA) was developed to perform stapled transanal rectal resection (STARR) . In 2009, its use was extended to endometriotic nodules involving the low and mid rectum . The procedure requires paring the nodule attached to the rectum during an initial laparoscopical step, resulting in the removal of 80–90% of the lesion.…”
