Combined pulmonary fibrosis and emphysema in a tyre industry worker

Abstract: We report a case of combined pulmonary fibrosis and emphysema (CPFE) with severe pulmonary hypertension in a 46-year-old man, nonsmoker, tyre industry worker. CPFE is commonly reported to be associated with tobacco smoking. This case highlights the possible role of environmental dust exposure (talc) in the pathogenesis of the disease and confirms the clinical characteristics of CPFE described in previous studies.

“…Interestingly, there is a growing recognition of coexistence of both emphysema and lung fibrosis in patients with chronic lung injury (1)(2)(3)(4)(5)(6)(7)(8). The majority of these patients have a history of either acute toxic gas inhalation or prolonged cigarette smoking, and present with very severe chronic obstructive pulmonary disease (COPD) with clinical and imaging features of both lung fibrosis and emphysema (4,9,10). Cigarette smoke-induced COPD characterized by increased collagen deposition around bronchioles and emphysema have also been reported in mice and ferrets (11)(12)(13)(14).…”

“…The role of environmental exposure as a potential trigger of lung injury is also plausible, given that some CPFE patients have had significant exposure to agrochemical compounds that cause airway damage and ILD in genetically susceptible smokers. ( 14 ) Some authors have described CPFE as an occupational disease, e.g., in patients exposed to talc ( 15 ) and in welders. ( 16 )…”

“…Some CPFE patients have had significant exposure to agrochemical compounds, which can result in interstitial lung disease and airway damage in normally genetically susceptible smokers [ 6 ]. Others have also described CPFE in patients with particular occupational diseases such as those exposed to talc [ 7 , 8 ] and welders [ 8 , 9 ]. Our patient, who is a non-smoker, states she worked at a hospital particularly the pharmacy department and played a role in delivering medications to the wards.…”
