Combined Off-Pump Coronary Revascularization and Lung Resection

Abstract: Concomittant severe coronary artery disease and lung malignancies occur rarely. Combined conventional coronary artery bypass grafting (CABG) with the use of cardiopulmonary bypass (CPB) with lung resection posses several perioperative and postoperative problems related to extracorporeal circulation and heparinization. The avoidance of CPB may be advantageous by decreasing blood loss, pulmonary complications and hospital stay. Further, exposure to the immunosuppresive and inflammatory effects of CPB may have de… Show more

“…The decreased "vigilance" of the immune system induced after the use of ECC can enhance tumor growth and accelerate its dissemination via the lymphatic system (18). It would be optimal to perform cardio-surgical procedures in patients with simultaneous lung cancer without the need of ECC, but that is possible in some patients who are scheduled for coronary surgery (19). Theoretically, the optimal solution, after taking in consideration the above mentioned, is to perform lung resection and subsequently cardiac surgery in the second period.…”

“…3. Considering the above mentioned reasons, some departments try to cover both of the systems together using the surgical therapy (6,10,18). The possibilities of surgical approach in combined disease of the heart and lungs are various.…”

“…The psychological effect is certainly essential for the patients who know about the tumorous proliferation and are waiting for the therapy. Another medical advantage is the possibility to avoid the effects of the cardiopulmonary bypass on the organism suffering from oncological disease (8,16,18,19). An indisputable advantage of median sternotomy is the possibility to perform bilateral mediastinal lymphadenectomy, which is important not only for the therapy, but for the prognosis.…”

“…The incidence of mortality and morbidity varied with each technique. Patients who underwent off-pump bypass had a 0% mortality 30,[32][33][34][35][38][39][40]42,55,57 and 0% to 48% morbidity 30,[32][33][34][35]39,40,42,55 in comparison to 0% to 20.8% and 0% to 67% among patients who underwent conventional bypass, respectively. 22 24 10 CCAB 5 (56%) -Takahashi T, et al (1995) 27 19 CCAB -5 (26%) Rao V, et al (1996) 28 24 CCAB 1 (4.5%) -Voets AJ, et al (1997) 51 34 CCAB 5 (18%) - Danton MHD, et al (1998) 29 Patients who underwent off-pump technique were found to be associated with lower incidence of blood loss, re-exploration for bleeding, cardiac and pulmonary complications, tumor recurrence, and death.…”
