Combined MitraClip and Left Atrial Appendage Occlusion: Is It Still a Utopia?

Abstract: Atrial fibrillation (AF) is the most common cardiac arrhythmia, affecting 32 million individuals worldwide, particularly the elderly. It is the main cause of ischemic strokes. Oral anticoagulation (OAC) is the gold standard strategy for stroke prevention. Still, there is a not negligible share of patients who have contraindications to this therapy, more frequently due to an increased risk of bleeding. AF is often associated with moderate-severe mitral regurgitation (MR), the second most frequent valvular disea… Show more

“…The aforementioned OPTION trial randomized 1600 patients, approximately one-third of which have LAAC device implanted concomitantly after ablation, and the results are anticipated in 2024. Belli et al 55 also shared their clinical experience of the combined approach of LAAC and MitraClip procedure with 3D echocardiography and new methods of volume imaging, which seems convincible and attracting. However, it is not recommended to routinely perform combined procedures with LAAC until more data on the efficacy and safety are available.…”

“…Therefore, combined percutaneous procedures are important in the field of interventional cardiology. [1] Atrial fibrillation (AF) is the most common cardiac arrhythmia, affecting 32 million people worldwide. Its incidence increases with age, reaching approximately 6% in people >60.…”

“…A registry collecting data on outcomes for patients undergoing simultaneous LAAC and transcatheter mitral valve edge-to-edge repair is currently ongoing ( NCT00494347 ). 3 However, the need for future TMVR cannot always be foreseen at the time of LAAC. With multiple devices under investigation for TMVR, consideration of the size and placement of a LAAC device and its possible interaction with future TMVR therapy is gaining importance.…”
