Combined Intra-Articular and Varus Opening Wedge Osteotomy for Lateral Depression and Valgus Malunion of the Proximal Part of the Tibia

Abstract: A knee-joint-preserving osteotomy can provide satisfactory results in active patients with painful posttraumatic lateral depression and valgus malunion of the proximal part of the tibia.

“…It is indicated to treat a valgus malunion of the metaphysis [5,18,24] but may not adequately compensate for severe depression of the lateral tibial plateau. Kerkoffs et al [13] recently published a technique combining osteotomies of the tibial metaphysis and articular surface to correct and unload malunions of the lateral compartment. This technique may be useful in preventing the development of posttraumatic arthritis or preventing its progressing in mild cases but is contraindicated in cases of severe osteoarthritis [13] such as those in our series.…”

“…These osteotomies result in relief of pain in 77% to 91% of patients [4,5,18] and restoration of function in 72% to 88% at 9.4 to 11.2 years [4,5,18]. In severe cases, it can be difficult, if not impossible, to correct a large intraarticular defect through an extra-articular procedure [13]. When there is full loss of cartilage in the affected compartment, osteotomy is usually no longer considered.…”

“…Lateral open-wedge high tibial osteotomies are rarely performed. There is a paucity reports regarding the use of a reconstructive osteotomy to treat depression and valgus malunions of the proximal tibia (Kerkhoffs et al, 2008). Barrel-vault (dome) osteotomies, lateral-closed-wedge, medial open-wedge high tibial osteotomies or a procedure that combines these techniques are used to treat varus deformities of the knee (Coventry, 1965;M Galla & Lobenhoffer, 2004;Hernigou et al 1987;Maquet, 1976;Nagi et al, 2007;Watanabe et al, 2008;Weber & Wörsdörfer, 1980).…”

“…The intra-articular osteotomy [2][3][4] can also be performed through the openingwedge tibial plateau osteotomy with a small bone distractor in situ. For this approach, including the elevation of the depressed lateral tibial plateau, it is helpful to create a small metaphyseal cortical window at the site of the tibial plateau osteotomy (Figs.…”

“…The operation is completed with the impaction of wedged corticocancellous autograft bone into the open gap and internal fixation with an L or a T-plate [2][3][4] . After extending the approaches, the tibial plate is usually sufficient to be used to fix both the Gerdy tubercle and the proximal tibial varus osteotomy at the same time.…”
