Objective To study the role of hysterolaparoscopy in the evaluation and management of female infertility.Materials and Methods A retrospective study of the 94 case files of all the patients who underwent diagnostic hysterolaparoscopy for infertility between January 2014 to June 2015 in the department of Obstetrics and Gynecology, Karnataka Institute of Medical Sciences, Hubli. These infertile women were confirmed to have normal ovulatory cycles, hormonal assays and seminogram report. Dye studies as well as inspection for abnormal pelvic and intrauterine pathology and necessary therapeutic interventions were done during the procedure. Abnormal pelvic and intrauterine pathology by hysterolaparoscopy were categorized. Results Out of 94 cases, 53.1 % patients had primary, 17.1 % patients had secondary infertility, and 29.8 % came for tubal recanalization. As a whole pelvic pathology was confirmed in 51.7 % and intrauterine pathology in 18.1 % patients by hysterolaparoscopy. The most common laparoscopic abnormality detected was ovarian pathology (20.8 %), followed by pelvic inflammatory disease 123(17.5 %). Tubal block comprised 7.7 % whereas distorted uterus by fibroid in 6.6 % and pelvic endometriosis in 5.4 %. In hysteroscopy, the incidence of uterine anomaly was 13 (13.8 %). Septate uterus is the most common with a mean incidence of approximately 7 (53.8 %). Conclusion Diagnostic hysterolaparoscopy is an effective diagnostic and therapeutic modality for certain significant and correctable abnormalities in pelvis, tubes and uterus which are missed by other imaging modalities.

Background: Infertility, as defined by World Health Organization (WHO), is failure to achieve pregnancy during 1 year of regular unprotected intercourse. The objective of this study was to determine the incidence of unsuspected pathology at hysterolaparoscopy in presumed unexplained infertility, the incidence of intervention done for correcting pathology and its outcome and the importance of hysterolaparoscopy in the evaluation and treatment of infertile couples.Methods: This prospective observational study was carried out at IVF and Endoscopy centre, Department of obstetrics and gynecology at the Ruby Hall clinic, Pune from 1st November 2014 to 30th July 2016, after obtaining institutional ethical clearance and who met the inclusion and exclusion criteria. A detailed clinical history and physical examination and bimanual pelvic examination were done, following which all the patients were subjected to baseline blood investigation, 3D pelvis (TVS) and semen analysis. Day care hysterolaparoscopy was performed and systematic analysis were done.Results: The mean duration of infertility was 2 to 4 years. In our study out of 75 women 62 (82.67%) showed normal hysteroscopic findings, remaining 13 women (17.67%) showed abnormal hysteroscopic findings like cornaul blockage, intrauterine adhesions and tuberculosis endometrium. Abnormal laparoscopic findings were reported in 29.33% of which the most common pathology was endomertiosis (21.33%).Conclusions: Diagnostic Hysterolaproscopy is a safe, effective, minimally invasive, cost effective, daycare comprehensive procedure in evaluation of unexplained infertility. Apart from routine diagnostic protocol missed pathologies can be detected and this tool can be used for diagnostic as well as therapeutic intervention.

A Am ma aç ç: : Ovülasyon indüksiyonu (Oİ) ve intrauterin inseminasyon (IUI) uygulanan infertil kadınlarda tanısal histeroskopinin gebelik oranlarına etkisini değerlendirmek. G Ge er re eç ç v ve e Y Yö ön nt te em ml le er r: : Bu çalışmaya ilk kez gonadotropin (GND) ve IUI döngüsüne giren 83 düşük riskli infertil hasta dahil edilmiştir. Obezite, komorbidite varlığı ve pelvik cerrahi öyküsü dışlama kriterleri idi. Anovulasyon ve açıklanamayan infertilite dışında diğer infertilite etiyolojisi olan ve histerosalpingogram (HSG) ve/veya ofis histeroskopi (HS) de herhangi bir uterin ve/veya tubal patoloji tanısı konan hastalar da çalışma dışı bırakıldı. En az bir siklus klomifen sitrat (CC) tedavisi almış olan hastalara GND tedavisinden önce tanı amaçlı HS önerildi. Tanısal HS yapılan 38 hasta (çalışma grubu) ile 43 hastanın (kontrol grubu) klinik özellikleri ve tedavi başarısı karşılaştırıldı. Patients with other infertility etiology except for anovulation and unexplained infertility, and who were diagnosed with any uterine and/or tubal pathology on hysterosalpingogram (HSG) and/or office hysteroscopy (HS) were also excluded from the study. After initial treatment with at least one cycle clomiphene (CC) therapy, a diagnostic HS was suggested to the patients to be performed before gonadotropin treatment. The clinical characteristics and treatment success of 38 patients (study group) who underwent diagnostic hysteroscopy and 43 patients (control group) who did not were compared.
