Abstract: Combined heart and lung transplantation is a technically feasible treatment for highly selected patients with localized advanced primary cardiac sarcomas. The high incidence of metastatic disease, however, limits its utility.

“…The data regarding cardiac sarcoma and outcome post-Tx are scarce and are not conclusive regarding the feasibility of cardiac Tx as a treatment option. 8,9 Emerging experimental and clinical short-term data have demonstrated anti-neoplastic properties of proliferation signal inhibitors. 20,21 However, only 1 patient was treated with proliferation signal inhibitors in our cohort, from which we cannot draw any conclusion.…”

“…4,5 Only small series with short observation times and without detailed analysis of the importance of the duration of pre-operative cancer-free interval have been published. [6][7][8][9][10][11][12] Hence, our aim was to investigate the influence of pre-Tx malignancy on post-Tx recurrence and long-term survival, focusing on preoperative cancer-free intervals in lung and heart Tx recipients.…”

“…Talbot et al [93] investigated the role of combined heart and lung resection followed by en bloc heart and bilateral lung transplant, in four patients where the sarcoma extended beyond the cardiac borders of cardiectomy. All patients had neoadjuvant chemotherapy.…”

“…Diagnosis with EBUS has the advantage of facilitating neoadjuvant therapy and hence reserving surgery for therapeutic purposes as the final step in multimodality treatment. Cardiac transplantation is another feasible therapeutic modality, but this approach would be disappointing due to local recurrence or metastases [13]. …”
