Combined evaluation of regional coronary artery calcium and myocardial perfusion by 82Rb PET/CT in the identification of obstructive coronary artery disease

Abstract: Hyperemic MBF and CFR provide incremental information about the presence of CAD over CAC score and perfusion imaging parameters. The combined use of CAC, myocardial perfusion imaging and quantitative coronary vascular function in may help predict more accurately the presence of obstructive CAD.

“…Indeed, it has been clarified that, on top of perfusion, details on hyperemic myocardial blood flow and coronary flow reserve may improve diagnostic and prognostic accuracy of PET. 40 Given the ongoing improvements in technology, procedures, and protocols, PET has reached favorable results in terms of sensitivity and specificity in comparison to ICA, with figures, respectively, of 84% and 81%. Similar results were found for FFR, with sensitivity reaching 86% and specificity 88%.…”

“…A preserved MFR is associated with low probability of coronary atherosclerosis and low risk of cardiac events; on the contrary, a reduced MFR is a significant marker that can help in stratifying CAD risk. [5][6][7][8] However, its diffuse application in the clinical scenario is limited by the technical complexity and elevated operative costs. 9 Previous studies investigated the feasibility of MFR quantification using dynamic scintigraphy and 99m Tc tracers in humans.…”

“…The peculiar advantage of hybrid PET combined with CT is the possibility to perform absolute quantification of coronary vascular function and coronary artery calcium, beyond myocardial perfusion evaluation as a part of the same examination. 21 Vulnerable plaque characteristics are associated with hemodynamically significant coronary artery lesions and a combined evaluation of morphological and functional characteristics of vulnerable plaque might provide better identification of culprit lesions with an incremental prognostic definition.…”
