Combined endothelial and epithelial barrier disruption of the colon may be a contributing factor to the Ebola sepsis-like syndrome

Abstract: After an initial febrile viral syndrome, infection with Ebola virus often induces an explosive late “Ebola sepsis-like syndrome” which appears very similar to some phenotypes of bacterial sepsis and is commonly fatal. It is possible that direct and diffuse viral infection of both the endothelium and epithelium of the colon may cause sufficient disruption of both the endothelial and epithelial barriers to induce exposure or leakage of endotoxin and bacterial antigens to, or into, the vascular system precipitati… Show more

“…Recently, intramedullary nailing has been considered the gold standard for the treatment of tibial shaft fractures [2], because it can provide good fracture reduction and early weight-bearing exercise. More importantly, intramedullary nail involves minimal surgical injury for preservation of blood supply to the fracture [3]. However, there are some disadvantages in traditional intramedullary nailing.…”

“…It carries no risk of damaging the patellar tendon and infrapatellar nerve, so it may reduce the rate of anterior knee pain [5]. This method helps to reduce varus and valgus deformities because the semi extended position may eliminate the extension force of the quadriceps [3]. The suprapatellar approach is controversial for it may lead to injury to the joint involvement and the cartilage.…”

“…1113 Furthermore, the application of this approach increases patients’ confidence in the final decision and enhances both patient and physician comfort in proceeding with surgery involving potential risks. 14, 15 Thus, patient concerns, preferences, and the perceived impact of treatment on quality of life must be considered when discussing treatment options to improve patient satisfaction with the shared treatment decision.…”

“…However, no signs of cardiogenic shock, severe chemical transaminitis corresponding to hepatic ischemia or necrosis, or severe hemolysis were observed at 5 dpi in EBOV Kikwit–infected NHPs or at 7 dpi in EBOV Makona–infected NHPs. One possibility is that the NHPs were experiencing either intestinal and/or mesenteric ischemia or necrosis, conditions that have been seen in other viral infections ( 39 , 40 ) and which may, as postulated by Lynn ( 41 ), precipitate late Ebola sepsis-like syndrome.…”
