Combined double superficial circumflex iliac artery perforator flap with lymphatic tissue preservation and lymphovenous anastomosis for lymphatic sequelae prevention in thigh defect reconstruction: A case report

Abstract: The thigh region is often site of soft tissue tumors development. Leiomyosarcoma, in particular, is a malignant tumor that, if not promptly treated, presents a growth rate that often results in large masses. The safest treatment in these cases is margin-free extensive surgical resection. This leads to rather large defects that in a delicate region, such as the medial thigh, implies a series of possible complications from the lymphatic point of view. In this region run major lymphatic vessels, appointed to drai… Show more

“…Initially presented as a strategy to prevent lymphedema or lymphatic complications during primary cancer surgery, 9 30 31 32 33 34 trans-flap LVAs have recently proposed for soft tissue reconstruction and treatment of lymphatic leakage and lower limb lymphedema. 23 Some surgeons may argue with the application of the flow-through terminology to the lymphatic deviation within the flap.…”

“…The SCIP flap is another very interesting technique in this region, where it proved its reliability in most different settings, including genital reconstruction (Scaglioni, Meroni, & Fritsche, 2022). It offers a series of interesting features: it is relatively quick to harvest, it can provide either a thin and very pliable amount of tissue or a rather large and thick island, the aesthetic result is usually good, and the donor site morbidity is very low (Chao et al, 2016; Scaglioni, Meroni, Fritsche, & Fuchs, 2022). The PMT flap, mostly supplied by the profunda femoris artery perforators (Scaglioni et al, 2015), also showed excellent results in either the perineal or upper thigh region (Scaglioni et al, 2017).…”

“…Thus, the edema results from disruption to both the lymphatics and the venous circulation and can be further exacerbated by adjuvant radiotherapy. Methods proposed to enhance lymph flow recovery following tissue reconstruction have the placement of the flap in an orientation where pre‐mapped lymphatic channels align axially with the lymphatic flow present at the defect site prior to resection, either with or without a lymphaticovenous anastomosis (Pereira et al, 2021; Scaglioni et al, 2021, 2022; Yamamoto et al, 2018, 2021). No lymphatic mapping was done prior to flap reconstructions in our patients.…”
