Combination technique of radiofrequency ablation with sclerotherapy in acquired lymphangiectasis of the vulva

Abstract: Aquired lymphangiectasis of the vulva is an uncommon condition and only few cases have been reported following tuberculous lymphadenitis. A case is reported that was successfully treated with a unique combination therapy of radiofrequency ablation and sclerotherapy with polidocanol. There was no recurrence at two years of follow-up.

“…[ 22 ] The combination of radiofrequency ablation with sclerotherapy was found to be simple, efficacious and cost-effective with a long-term satisfactory results. [ 23 ] Gianelli and Rockley found electrodessication superior to the other treatment modalities. [ 4 ] Each therapeutic option is effective, but recurrences are common and may be complicated by scarring.…”

“…Polidocanol, a compound first developed as a long‐acting local anesthetic, 15 is currently used most widely in human medicine as a sclerosing agent. Reported use was successful in the treatment of varicose veins of the lower extremities, 16–18 telangiectasias, 16 capillary malformations associated with Klippel–Trenaunay syndrome, 19 acquired lymphangiectasis of the vulva, 20 hydroceles and epididymal cysts, 21 and other vascular anomalies. Use in animals has been reported infrequently, but 1% polidocanol was also shown, experimentally, to address spontaneous pneumothorax and hydrothorax by having a potent sclerosant effect on the mesothelium of the porcine pleura by achieving pleurodesis 15 .…”

“…Complications with the use of 1% polidocanol are reported infrequently in the human literature. The most common adverse effects include pigmentation at the injection site or immediate and transient local swelling, pain, and ulceration when superficial injection into the dermis occurs 20 . Very rare complications including allergic reaction or deep venous thrombosis have been reported 18 .…”
