The sparing effects of tramadol and tramadol-lidocaine infusion on the minimum alveolar concentration (MAC) of sevoflurane in dogs as well as the entropy indices were investigated. Anesthesia was induced in eight young, healthy German shepherds weighing 27.6 ± 3.2 kg (mean ± SD) and maintained with sevoflurane. A standard tail-clamp technique was used to determine sevoflurane MAC during infusion with: sevoflurane alone to measure baseline MAC (MAC B ); tramadol (intravenous loading dose of 1.5 mg/kg and constant rate infusion [CRI] of 2.6 mg/kg/hr; MAC T ); and tramadol-lidocaine (tramadol CRI of 2.6 mg/kg/hr; and lidocaine intravenous loading dose of 1.0 mg/kg and CRI of 6 mg/kg/hr; MAC TL ). The state entropy (SE), response entropy (RE), and RE-SE difference were recorded 5 min prior to and during tail clamping. MAC B was 2.4 ± 0.2%. Tramadol and tramadol-lidocaine CRI decreased MAC to 2.2 ± 0.3% and 1.7 ± 0.3%, respectively. The MAC-sparing effect of tramadol-lidocaine was greater than that of tramadol alone (8.2 ± 8.9% vs. 30.1 ± 10.7%; P <0.01). SE and RE in all subjects, and RE-SE difference in most subjects, were increased (all P <0.05) when they responded purposefully to noxious stimulation. A tramadol-lidocaine combination infusion can reduce anesthetic requirements to a higher degree than tramadol alone. Furthermore, MACentropy, MAC required to prevent increased entropy in response to a painful stimulation, and MAC of sevoflurane were similar in dogs.

IntroductionThe objective of this study is to make a comparison between the effectiveness of the diclo- fenac suppository alone and periprostatic local anaesthesia infiltration alone to reduce the pain during a transrectal ultrasound-guided prostate biopsy procedure.Material and methodsBetween January 2014 and December 2015, 100 patients from two centres who were scheduled for transrectal ultrasound guided prostate biopsy (TRUS Bx) were included in the study. Patients were divided into two groups; diclofenac suppository group (Group 1) and Lidocaine group (Group 2). After the prostate biopsy, patients were requested to fill out a visual analogue scale in order to evaluate their pain level during the procedure.RESULTSSince each group had been examined for VAS score, statistical differences were detected for VAS 0 and VAS 1. VAS 0 score was stated in 38 (71%) patients in Group 2, and in 25 (50%) patients in Group 1 (p = 0.040). VAS 1 score was stated in 9 (17%) patients in group 2 and 19 (38%) patients in Group 1 (p = 0.030).CONCLUSIONSAlthough statistical difference was detected at lower VAS scores (0 and 1) in favor of the lidocaine group during transrectal ultrasound guided prostate biopsies, there was not a significant difference in higher (>2) VAS scores; which was made with 100 mg of diclofenac suppository. Therefore, diclofenac suppository can be used as an alternative to periprostatic nerve block made with lidocaine.

Introduction: The study aimed to compare the efficiency of periprostatic nerve blockage (PPNB) and intrarectal lidocaine gel (PPNB + gel) with a transperineal prostatic block (TPPB) technique during transrectal ultrasound-guided prostate biopsy (TRUS-PBx) in patients with anorectal pathologies. Materials and Methods: A total of 376 patients who underwent TRUS-PBx were randomized into 2 groups. Group-I (n = 198) received TPPB with 10 ml 2% prilocaine, and group-II (n = 178) received intrarectal administration of 10 ml 2% lidocaine gel followed by PPNB with 10 ml 2% prilocaine. A 10-point linear visual analogue scale (VAS) was used to assess the pain arising from probe insertion (VAS-1) and prostate sampling (VAS-2). Results: VAS-1 scores were significantly lower in group-I than group-II (1.7 ± 1.9 vs. 3.9 ± 1.5; p < 0.001). Combining local anesthesia produced superior pain control to TPPB during sampling (2.0 ± 1.2 vs. 2.5 ± 2.4; p = 0.015). Following subgroup analyses with reference to concomitant anorectal pathologies, VAS-1 scores were significantly lower in group-I than group-II (2.0 ± 1.8 vs. 5.5 ± 1.7; p < 0.001). VAS-2 scores were lower in group-II than group-I; however, the difference was not considered significant (2.4 ± 1.3 vs. 3.1 ± 2.8; p = 0.303). Conclusions: In all patients referred for TRUS-PBx, TPPB is a good alternative to PPNB + gel. TPPB can be particularly useful for patients with anorectal pathologies due to its improved pain reduction during probe insertion.
