Abstract: AIM: The aim of the study is to analyze the effi ciency of combination of RBPT, STAT and ELISA tests on patients with brucellosis where the diagnosis is dependent on the compatible clinical and epidemiological fi ndings with negative RBPT and/or STAT test results. ÖzBrusellosis ülkemizde endemik olarak görülen bir zoonotik hastalıktır ve sıklıkla pastörize olmamış süt ve süt ürün-lerinin tüketilmesi ile bulaşır. Nörobrusellozis hastalığın ciddi bir komplikasyonudur ve hastalığın herhangi bir safhasında gelişebilir. Nörobrusellozda klinik spektrum santral ve periferal olarak sınıflandırılır. En sık görülen klinik tablolar menenjit, meningoensefalit, myelit ve kraniyal sinir paralizileridir. Nörobruselloz vakalarında psikiyatrik tablolar (depresyon, amnezi, psikoz ve ajitasyon) rapor edilmiştir. Beyin omurilik sıvısının incelemesinde protein konsatrasyonu artmış, glukoz konsantrasyonu azalmış ve başlıca lenfositlerden oluşan orta derecede lökosit artışı mevcuttur. Brusella türlerinin beyin omurilik sıvısından izolasyonu düşük olduğundan nörobruselloz vakalarına en sık serolojik yöntemlerle tanı konulmaktadır. Tedavide doksisiklin, rifampisin, siprofloksasin, seftriakson ve trimetoprim-sülfame-taksazol tercih edilir. Etkili tedavi ile sinir sistemi hasarına bağlı sekel azalır. Anahtar kelimeler: Nörobruselloz, klinik tablolar, tedavi AbstractBrucellosis is an endemic zoonotic disease in our country and it is frequently transmitted to humans by ingestion of unpasteurised milk or milk products. Neurobrucellosis is a serious complication of brucellosis and may develop at any stage of disease. The clinical spectrum of neurobrucellosis classified as central and peripheral. The most frequent clinical presentations are meningitis, meningoencephalitis, myelitis and cranial nerve paralyses. The psychiatric manifestations (depression, amnesia, psychosis and agitation) were reported in neurobrucellosis cases. Examination of the cerebrospinal fluid an elevated protein concentration, a depressed glucose concentration and a moderate leukocytosis compodes mainly of lymphocytes. The isolation rate of Brucella species from cerebrospinal fluid is so low, most of the neurobrucellosis cases were diagnosed by serological methods. Doxycyclie, rifampin, ciprofloxacin, ceftriaxone and trimethoprim-sulfamethoxazole preferred in the treatment of neurobrucellosis. Effective treatment reduce the sequelae caused by nervous system damage.

Amaç Bruselloz; Brucella türlerinin sebep olduğu zoonotik bir hastalıktır. İnsanlarda gelişen bruselloz tanısında başlıca kültür ve serolojik yöntemler kullanılmaktadır. Biz hastanemizdeki bruselloz serosürveyansını belirlemeyi amaçladık. Materyal ve MetodTemmuz 2014-Temmuz 2019 tarihleri arasında hastanemiz mikrobiyoloji laboratuvarına gönderilen bruselloz ön tanılı hasta örnekleri Rose Bengal testi (Sero-Lam Brusella Rose Bengal Plate Test Seromed, Türkiye) ve Brucella Coombs jel testi (ODAK Brucella Coombs Gel Test, İstanbul, Türkiye) ile incelenmiştir. Rose Bengal testinde ha f ya da belirgin kümelenme görülmesi "pozitif " olarak değerlendirilmiştir. Brucella Coombs jel testinde ise; 1/160 ve üzerindeki titreler pozitif olarak kabul edilmiştir. Bulgular Çalışmada incelenen bruselloz şüpheli 13796 hasta örneğinden 1040 (%7,5) ında Rose Bengal testi pozitif olarak bulunmuştur. Brucella Coombs jel testi pozitif olan hasta sayısı ise 468 (%3,4) dir. 615 (%4,5) serum örneğinde Rose Bengal testi pozitif olup, Brucella Coombs jel testi negatif saptanmıştır. 43 hasta örneği ise Rose Bengal testi ile negatif iken Brucella Coombs jel testi ile pozitif olarak belirlenmiştir. Sonuç Rose Bengal ve Brucella Coombs jel test kullanılarak hastanem zde son 5 yıldak Brusella serosürveyansını araştırdık. Son yıllarda kullanımı yaygınlaşan Brucella Coombs jel test le elde ett ğ m z sonuçların b rçok laboratuvarda kullanılan Standart tüp aglüt nasyon test le uyumlu olduğunu özell kle blokan ant korlar varlığında sonuçların daha güven l r olab leceğ n düşünmektey z.
