BackgroundWarts are one of the most common infections in humans. Plantar warts are a subtype of non-genital warts, and several procedures and topical treatments have been used in its treatment. Cryotherapy is one of the most popular modalities, but it is time-consuming and remission rates vary in different studies. CO2 laser was the first laser used for treating warts. To date, no clinical trial has been done to compare CO2 laser with cryotherapy in the treatment of plantar warts.Patients and MethodsThis randomized controlled trial was performed in order to compare the efficacy and number of sessions needed to treat plantar warts in 60 patients who had received no previous treatment in the previous 3 months. They were randomly allocated to the cryotherapy or CO2 laser group. The number of sessions needed for response and the recurrence rate after a 3-month follow-up was compared in the 2 groups.ResultsSixty patients with plantar warts were randomly allocated to either the CO2 laser or cryotherapy groups. Median age was 25 (range=18–53) and 27 (range= 18–75) years in the cryotherapy group and CO2 laser groups, respectively. Both groups were matched for age and sex (56% male and 44% female in the cryotherapy group and 34% male and 66% female in the CO2 laser group). The median number of sessions needed for complete resolution of the warts in the CO2 laser and cryotherapy groups were 1 (range=1–2) and 3 (range=1–12), respectively. The difference in the number of sessions was statistically significant between the 2 groups (P-value≤0.001). Recurrence rates after a 3-month follow-up was not statistically significant (P-value= 0.069).ConclusionThe number of sessions needed to treat plantar warts was less using CO2 laser than cryotherapy; therefore, this modality can be a good addition to the already existing anti-wart armamentarium.

ÖzetEstetik düzenlemeler için geliştirilen cerrahi olmayan lazerler ile cilt yenileme alanı, yeni araştırmalar ve teknik ilerlemelerle gelişimini devam ettirmektedir.1980'lerdeki ilk kullanımlarından bu yana lazerle cilt yenileme endüstrisi ablatif, nonablatif ve fraksiyonel ablatif özellikte çok sayıda cihaz geliştirmiştir. Ablatif cilt yenileme, yaşlanmış derinin rejuvenasyonu ve çok sayıdaki deri lezyonunun tedavisi için oldukça güçlü bir araçtır. Uygun ellerde kullanıldıklarında, cerrahi ve nonablatif metodların tavsiye edilmediği fotoyaşlanma problemlerinin çoğunda etkili ve güvenli bir yöntemdir. Ablatif lazerler, ürettikleri infrare ışık sayesinde "su" molekülünü hedefleyerek, doku kontraksiyonu ve kollajen üretimine neden olurlar. İlk ablatif lazer sistemleri, belirgin termal hasar oluşturduklarından kabul edilemez yüksek oranlarda skarlaşma ve uzamış yara iyileşmesine neden olmuşlardır. Yüksek enerjili pulse lazerler ve fraksiyonel lazerler gibi yeni cihazlar, daha az yan etki ve kıssa yara iyileşme süreleri ile belirgin düzelmelere ulaşabilme kapasitesine sahiptirler. Ablatif cilt yenileme giderek daha güvenli olmakla beraber, tedavi sonrası skarlaşma, pigment düzensizlikleri ve enfeksiyonları engellemek için dikkatli ve uygun hasta seçimi halen çok önemlidir. Sum maryThe field of nonsurgical laser resurfacing for aesthetic enhancement continues to improve with new research and technological advances. Since its beginnings in the 1980s, the laser-resurfacing industry has produced a multitude of devices employing ablative, nonablative, and fractional ablative technologies. Ablative resurfacing is a powerful tool for rejuvenation of the aging face and for the treatment of a wide array of skin lesions. In the proper hands, it is a safe and effective way to treat many of the problems of photodamaged skin that surgery or nonablative methods cannot address. Ablative lasers emit infrared light, which targets water resulting in tissue contraction and collagen formation. The first ablative laser systems created significant thermal damage resulting in unacceptably high rates of scarring and prolonged healing. Newer devices, such as high-energy pulsed lasers and fractional ablative lasers, are capable of achieving significant improvements with fewer side effects and shorter recovery times. While ablative resurfacing has become safer, careful and appropriate patient selection is still important to avoid post-treatment scarring, dyspigmentation, and infections. Clinicians utilizing ablative devices need to be aware of possible side effects in order to maximize results and patient satisfaction.
