Background: The real accuracy of computed tomographic colonography (CTC) is still unknown. Objective: To perform a meta-analysis of the diagnostic accuracy of CTC for the detection of polyps and colorectal tumors. Methods: Selection of studies:Studiesassessing the accuracy of CTC for the detection of colorectal polyps and tumors were selected. Data synthesis:Meta-analyses combining sensitivities, specificities and likelihood ratios (LRs) for the diagnosis of polyps and colorectal tumors were carried out. Results: Forty-seven studies, providing data of 10,546 patients, were included. Overall per-polyp sensitivity of CTC was 66% (64–68%), for polyps 6–9 mm in size it was 59% (56–61%), and 76% (73–79%) for polyps larger than 9 mm. Overall per-patient sensitivity was 69% (66–72%), for polyps 6–9 mm 60% (56–65%), and 83% (70–85%) for lesions larger than 9 mm. Overall CTC specificity was 83% (81–84%). Positive and negative LRs were 2.9 (1.8–4) and 0.38 (0.27–0.53), respectively; for polyps 6–9 mm in size, they were 3.8 (2.5–5.7) and 0.4 (0.27–0.59), and 12.3 (7.7–19.4) and 0.19 (0.12–0.3) for polyps larger than 9 mm. Conclusion: CTC is highly specific for the detection of colorectal polyps and tumors. Some studies reported high sensitivities, but the results of the studies were highly heterogeneous, while the studied variables explained only part of this discrepancy.

According to the statistical yearbook of the Ministry of Health and Welfare, crude colorectal cancer incidence rates in Korea in 2008 were 54.7 and 36.9 per 100,000 among males and females, respectively. Colorectal cancer is thus the second most common cancer in males and the fourth most common Clin Endosc 2012;45:25-43 Copyright © 2012 The Korean Society of Gastrointestinal Endoscopy 25 cancer in females. 1 The mortality rates (the number of deaths per 100,000 people, %) of colorectal cancer in 2008 were 15.4% among males and 12.1% among females. Colorectal cancer is thus the fourth most common cause of cancer death among Korean males and the second most common cause of cancer death among Korean females. The age-standardized mortality rates of colorectal cancer continue to increase, unlike those of other common cancers, including lung cancer, liver cancer and stomach cancer, whose mortality rates have decreased in recent years. Increases in the incidence of colorectal cancer may be partly due to Westernization of many Koreans' lifestyles. Greater public awareness of colorectal cancer and the addition of colorectal cancer in 2004 to the national cancer screening program are likely also responsible in part for the increased incidence rates. Participation rates in the national colorectal screening program increased from ap Now colorectal cancer is the second most common cancer in males and the fourth most common cancer in females in Korea. Since most of colorectal cancers occur after the prolonged transformation of adenomas into carcinomas, early detection and removal of colorectal adenomas are one of the most effective methods to prevent colorectal cancer. Considering the increasing incidence of colorectal cancer and polyps in Korea, it is very important to establish Korean guideline for colorectal cancer screening and polyp detection. The guideline was developed by the Korean Multi-Society Take Force and we tried to establish the guideline by evidence-based methods. Parts of the statements were draw by systematic reviews and meta-analyses. Herein we discussed epidemiology of colorectal cancers and adenomas in Korea and optimal methods for screening of colorectal cancer and detection of adenomas including fecal occult blood tests, radiologic tests, and endoscopic examinations.

Colorectal cancer (CRC) is an important social and medical problem. Complete recovery can be achieved only if the disease is diagnosed in the early stages, most common by screening methods. Studies have shown that screening methods reduce the incidence and mortality of colorectal cancer. The methods for colorectal screening can be divided into tests that allow the detection of cancer (fecal occult blood test and fecal DNA test) and morphological tests that allow diagnosis of cancer or adenomatous polyps (flexible sigmoidoscopy, colonoscopy, double contrast barium enema, CT colography ). In people with average risk of disease, screening begins at 50 years and performed on one of the following: fecal occult blood test (every year), flexible sigmoidoscopy (every 5 years), colonoscopy (every 10 years), double contrast barium enema (every 5 years) or CT colography (every 5 years). Colonoscopy is the preferred method, and is used as a confirmatory method, if the any other methods was positive. ApstraktKolorektalni karcinom je veliki socijalno-medicinski problem. Potpuno izlečenje se može postići jedino ukoliko se bolest dijagnostikuje u ranim stadijumima, što se postiže skrining metodama. Studije su pokazale da skrining metode smanjuju incidenciju i mortalitet od kolorektalnog karcinoma. Metode koje se koriste za skrining kolorektalnog karcinoma se mogu podeliti na testove koji omogućavaju detekciju karcinoma (test na okultno krvarenje i fekalni DNA test) i morfološke testove koji omogućavaju dijagnostiku karcinoma ili adenomatoznih polipa (fleksibilna sigmoidoskopija, kolonoskopija, irigografija, CT kolografija). Kod osoba sa srednjim rizikom od obolevanja, skrining započinje sa 50 godina i sprovodi se na jedan od sledećih načina: testom na okultno krvarenje u stolici (svakih godinu dana), fleksibilnom sigmoidoskopijom (svakih 5 godina), kolonoskopijom (svakih 10 godina), irigografijom (svakih 5 godina) ili CT kolografijom (svakih 5 godina). Kolonoskopija je preferirana metoda, a koristi se i kao potvrdna metoda, ukoliko je nalaz bilo koje druge metode pozitivan.

Abstract: In patients at average risk for colorectal cancer, CT colonography is a sensitive and specific screening test for detecting polyps 10 mm or larger; the sensitivity for detecting smaller polyps is decreased. Examination findings can be interpreted in a clinically feasible amount of time.
