“…When all the decisions on a patient’s treatment sequence have been taken, usually at the point when the definitive histology of tumour resection has been received and post‐operative adjuvant therapy has been determined, we ‘sign off’ these people from our MDT and record the nature of and mode of planned follow‐up, e.g. ‘Follow‐up will be by nurse‐led surveillance for 5 years,’ or ‘She will be seen once in OPD and then discharged back to Primary Care.’ Currently we are succeeding in specifying the mode of follow‐up at ‘sign‐off’ in 83% of our patients [2].
