Εισαγωγή: Ο σωστός χαρακτηρισμός των ωοθηκικών βλαβών είναι κρίσιμος για τη βελτιστοποίηση της περίθαλψης του ασθενούς. Σκοπός αυτής της μελέτης είναι η αξιολόγηση της διαγνωστικής απόδοσης του μαθηματικού μοντέλου παλινδρόμησης (LR1), του μοντέλου παλινδρόμησης (LR2) και του δείκτη κινδύνου κακοήθειας (RMI) για τον προεγχειρητικό χαρακτηρισμό των εξαρτηματικών βλαβών, όταν το υπερηχογράφημα εκτελείται από μη έμπειρο εξεταστή επίπεδο Ι-ΙΙ. Μέθοδοι: Μια 9ετής αναδρομική μονοκεντρική μελέτη στη μονάδα γυναικολογικής υπερηχογραφίας του Νοσοκομείου Αλεξάνδρα. Το διακολπικό υπερηχογράφημα πραγματοποιήθηκε χρησιμοποιώντας τυποποιημένη προσέγγιση. Το τελικό αποτέλεσμα ήταν τα χειρουργικά ευρήματα και η ιστολογική διάγνωση που ήταν ο ‘χρυσός κανόνας’. Για τον χαρακτηρισμό των εξαρτηματικών βλαβών, εφαρμόστηκε το μοντέλο πρόβλεψης με 12 μεταβλητές (LR1) με cutoff το 0,1, το μοντέλο πρόβλεψης έξι μεταβλητών (LR2) με cutoff το 0,1 και ο δείκτη κινδύνου κακοήθειας RMI με cutoff το 200. Η περιοχή κάτω από τις καμπύλες (AUC) για την απόδοση των LR1, LR2 και RMI υπολογίστηκε. Τα μέτρα διαγνωστικής απόδοσης για όλα τα μοντέλα που αξιολογήθηκαν ήταν η ευαισθησία, η ειδικότητα, ο θετικός και αρνητικός λόγος πιθανοφάνειας (LR + και LR -) και ο λόγοςπιθανοτήτων (DOR). Αποτελέσματα: Χίλιες τριάντα δύο (1032) γυναίκες με εξαρτηματικά μορφώματα υποβλήθηκαν σε διακολπικό υπερηχογράφημα, ενώ οι 792 από αυτές υποβλήθηκαν στησυνέχεια σε χειρουργική επέμβαση. Το ποσοστό κακοήθειας ήταν 14,6%. Οι τιμές AUC για το LR1, LR2 και RMI για το σύνολο των μορφωμάτων ήταν 0,886 (95% CI: 0,855-0,917), 0,839 (95% CI: 0,928). Η ευαισθησία των LR1, LR2, RMI στο σύνολο των μορφωμάτων ήταν 0,741, 0,724 και 0,667, ενώ η ειδικότητα των LR1, LR2 και RMI ήταν 0,854, 0,855 και 0,960. Τα δύο μοντέλα IOTA είχαν μεγαλύτερη ευαισθησία σε σύγκριση με το μοντέλο RMI. Στο σύνολο των μορφωμάτων, οι λόγοι πιθανοτήτων (DOR) για τα LR1, LR2 και RMI ήταν 16,707 (95% CI: 13,165-21,204), 15,482 (95% CI: 12,243-19,578), 47,846 (95% CI: 32,553-70,324). Συμπέρασμα: Αποτελεί την πρώτη μελέτη πολύ μεγάλου δείγματος στην Ελλάδα, όπου γίνεται σύγκριση του LR1 και LR2 IOTA μοντέλου με το γνωστό μέχρι σήμερα μοντέλο RMI. Τελικά, τα μοντέλα IOTA LR1 και LR2 λόγω της πιο υψηλής τους ευαισθησίας, φαίνεται να είναι ένα βασικό εργαλείο για την υπερηχογραφική αξιολόγηση της φύσης των ωοθηκικών μορφωμάτων στα χέρια των κλινικών γυναικολόγων ούτως ώστε να αναγνωρίζεται η εξαρτηματική παθολογία και ειδικά η κακοήθεια. Το μοντέλο RMI παραμένει ένα εξίσου σημαντικό εργαλείο για τη διάκριση της εξαρτηματικής παθολογίας.

Characterizing ovarian masses enables patients with malignancy to be appropriately triaged for treatment by subspecialist gynecological oncologists, which has been shown to optimize care and improve survival. Furthermore, correctly classifying benign masses facilitates the selection of patients with ovarian pathology that may either not require intervention, or be suitable for minimal access surgery if intervention is required. However, predicting whether a mass is benign or malignant is not the only clinically relevant information that we need to know before deciding on appropriate treatment. Knowing the specific histology of a mass is becoming of increasing importance as management options become more tailored to the individual patient. For example predicting a mucinous borderline tumor gives the opportunity for fertility sparing surgery, and will highlight the need for further gastrointestinal assessment. For benign disease, predicting the presence of an endometrioma and possible deeply infiltrating endometriosis is important when considering both who should perform and the extent of surgery. An examiner’s subjective assessment of the morphological and vascular features of a mass using ultrasonography has been shown to be highly effective for predicting whether a mass is benign or malignant. Many masses also have features that enable a reliable diagnosis of the specific pathology of a particular mass to be made. In this narrative review we aim to describe the typical morphological features seen on ultrasound of different adnexal masses and illustrate these by showing representative ultrasound images.
