Color and Pulsed Doppler Ultrasonography of Reversed Umbilical Blood Flow in an Acardiac Twin

Abstract: Prenatal diagnosis of an acardiac twin pregnancy was performed. At 30 gestational weeks, pulsed color Doppler ultrasound revealed polyhydramnios, reversed-pulsatile blood flow in the umbilical artery and vein of the acardiac twin, and artery-artery anastomosis on the placental surface. The total cardiac dimension, maximum blood flow velocity of the ascending aorta and the pulmonary artery in the alive twin were in the normal range.

“…An acardiac twin must be suspected in all monochorionic, malformed fetuses with cystic hygroma, generalized edema, and an absent cardiac pulsation with a nonfunctioning heart.Similiarly, an ultrasonography finding of twins revealing discordant or grotesque malformation along with reverse flow in the umblical artery is usually diagnostic of an acardiac twin (Ishimatsu et al, 1993). …”

“…A monochorionic, malformed fetus with cystic hygroma, generalized edema and an absent cardiac pulsation with a non-functioning heart on ultrasonography should raise suspicion towards the possibility of acardiac twinning.Sonography findings of discordant growth along with color Doppler finding of reverse flow in the umbilical artery is diagnostic of an acardiac twin. 5 But the diagnosis is not always simple. The most frequent confusion is with anencephaly or with fetal demise of one twin.…”

“…Компьютерная томография проведена для визуализации костного скелета, в том числе позвоночного столба, и для дифференциальной диагностики с тератомой, в ряде случаев имеющей вид аморфных масс. В отличие от тератомы, акардиальный плод всегда имеет связь с плацентой посредством пуповины [3,4], а также в большинстве случаев плод-акардиус имеет сформированный краниокаудальный скелет [5].…”
