Acardiac malformation, also known as twin reversed arterial perfusion (TRAP) sequence, is a unique complication of monochorionic twinning., occurring with a reported incidence of 1 in 35 000 deliveries. It is characterized by lack of heart development associated with a spectrum of malformations and anomalies in one of the twins, which is perfused in a paradoxical retrograde fashion by a structurally normal 'pump' twin through a single artery-to-artery anastomosis. There is usually a normally formed donor twin who has features of heart failure as well as a recipient twin who lacks a heart (acardius) and various other structures.It is caused in the embryo by a large artery-to-artery placental shunt, often also accompanied by a vein-to-vein shunt. The common complications include congestive cardiac failure in the pump twin, polyhydramnios and preterm delivery.

Twin reversed arterial perfusion (TRAP) sequence is an extremely rare complication of monochorionic multi-foetal pregnancy, occurring once in 35,000 births. It is characterized by a malformed foetus without a heart being perfused by a structurally normal (pump) twin via an artery-to-artery anastomosis in a reverse direction. We report one such case where ultrasound imaging revealed monochorionic twin pregnancy with a viable, normal-appearing first twin and a structurally aberrant second twin with absent cardiac activity. The patient was monitored with two weekly ultrasonography and Doppler ultrasound examination to ascertain the well-being of the pump twin. She delivered successfully at term a normal live baby and an acardius anceps foetus. The perinatal mortality of the pump twin ranges from 35 to 55%; making it essential to diagnose the presence of a pump twin at an early gestational age through improved imaging techniques to plan effective intervention at appropriate time.

The article describes an acardiac fetus in a patient with monochorionic diamniotic twin pregnancy with reversed arterial perfusion syndrome at 30 weeks' gestation. It gives postmortem fetal computed tomographic and pathoanatomic data. Microarray of acardiac fetal tissues revealed that there was deletion of chromosome 19 - arr [hg19] 19p13.3q11 (260,911-23,005,001) x1; size, 23 Mbp; the signal level in about 30% of fetal tissue cells containing deletion.
