Color and pulsed doppler sonography, grayscale imaging, and serum CA 125 in the assessment of adnexal disease

Abstract: Color and pulsed Doppler sonography, which demonstrate a tumor angiogenic activity, are as accurate as gray-scale imaging in the assessment of adnexal lesions. Together with serum CA 125 marker levels, they produce high negative predictive values, providing reassurance that an adnexal mass is benign.

“…In a study of 63 benign tumors, Prömpeler and coauthors 8 found a sensitivity in detecting benign tumors of 91% for a PSV ≥ 30 cm/s. 8 In a study that included seven malignant tumors, Predanic and coworkers 6 found the mean PSV to be 8.3 cm/s, with a sensitivity of 85%, a specificity of 71%, a PPV of 21%, and an NPV of 98% in detecting benign tumors. 6 In our study, the mean PSV in malignant tumors was 12.4 cm/s.…”

“…[4][5][6][7][8] Serum CA 125 concentrations can also be misleading-results may be falsely positive in women with conditions that affect the peritoneal surface, such as endometriosis and pelvic infection. 9 10 Because epithelial ovarian malignancy is rare in this age group, other tumour markers, such as α fetoprotein, lactate dehydrogenase, and serum β human chorionic gonadotrophin, should be investigated to rule out germ cell tumours.…”
