Use of unrelated donor cord blood (CB) as an alternative stem cell source is increasing, and yet there is little information to guide transplant centers in the unique aspects of the search and selection of CB grafts. There is no mechanism to easily access the global inventory of CB units, nor is the product information provided by all banks standardized. To address these challenges, this manuscript reviews the logistics of the search, selection process, and acquisition of CB grafts as practiced by our center. Topics include who should be considered for a CB search, how to access the global CB inventory, and how to balance total nucleated cell dose and human leukocyte antigen match in unit selection. We discuss aspects of unit quality and other graft characteristics (processing methods, unit age, availability of attached segments, infectious disease, and hemoglobinopathy screening) to be considered. We incorporate these considerations into a unit selection algorithm, including how to select double-unit grafts. We also describe how we plan for unit shipment and the role of backup grafts. This review aims to provide a framework for CB unit selection and help transplantation centers perform efficient CB searches. (Blood. 2011;117(8):2332-2339) IntroductionUnrelated donor cord blood transplantation (CBT) has become a widely accepted treatment for lethal hematologic diseases. Both the number of CB transplantations 1 and the global inventory of CB units (estimated at 400 000) 2 are growing rapidly. Therefore, it is critically important that transplantation centers (TCs) have a thorough understanding of how to perform a CB search as well as the challenges encountered in the selection and acquisition of CB grafts. This article is a practical guide for the TCs, especially those new to the field of CBT, and is based on our daily experience of searching the global CB inventory, our knowledge of CB banking and CB testing standards, and evaluation of recently published data. Thus, we outline our search practices at Memorial SloanKettering Cancer Center (MSKCC). Who should get a formal CB search with confirmatory HLA typing of CB unitsThere is a progressive decrease in post-transplantation survival with each human leukocyte antigen (HLA) or allele mismatch at HLA-A, -B, -C, -DRB1 loci of adult unrelated donor (URD) grafts, with donor-recipient HLA-DQ disparity being detrimental when present with other mismatches. 3 Therefore, TCs must decide what level of HLA disparity will be tolerated with a URD before alternative hematopoietic stem cell sources such as CB are sought. In addition, as CB grafts are available faster than URD, 4 transplantation urgency may be an additional reason to use CB. Summary Unrelated cord blood (CB) units, already manufactured, fully tested and stored, are high‐quality products for haematopoietic stem cell transplantation and cell therapies, as well as an optimal starting material for cell expansion, cell engineering or cell re‐programming technologies. CB banks have been pioneers in the development and implementation of Current Good Manufacturing Practices for cell‐therapy products. Sharing their technological and regulatory experience will help advance all cell therapies, CB‐derived or not, particularly as they transition from autologous, individually manufactured products to stored, ‘off‐the shelf’ treatments. Such strategies will allow broader patient access and wide product utilisation.

Ο αριθμός των μεταμοσχεύσεων με ΟΠ.Α προερχόμενο από HLA συμβατούς, μη συγγενείς, δότες έχει αυξηθεί σημαντικά. Η «συλλογή» των HLA φαινοτύπων στα κέντρα δοτών HSC και τις Τράπεζες ΟΠ.Α δίνουν την ευκαιρία στους αιματολόγους-μεταμοσχευτές από όλο τον κόσμο να πραγματοποιούν προκαταρκτικές έρευνες, μέσω κατά τόπους Εθνικών Οργανισμών Μεταμόσχευσης. Με στόχο την εύκολη διάθεση των μοσχευμάτων που περιέχονται στις δεξαμενές, οι Τράπεζες ΟΠ.Α αναρτούν τις πιστοποιημένες μονάδες τους στην Παγκόσμια Δεξαμενή BMDW, με τα ελάχιστα βασικά δεδομένα που απαιτούνται για μια διαδικασία αναζήτησης. Στο αποτέλεσμα μιας αναζήτησης, συνήθως εμφανίζονται οι υποψήφιες μονάδες ΟΠ.Α πρώτα με βάση τη συμβατότητά τους με τα HLA αντιγόνα του λήπτη και στη συνέχεια, βάσει του αριθμού των κυττάρων. Άλλες παράμετροι μπορούν επίσης να χρησιμοποιηθούν ως «φίλτρα» των αποτελεσμάτων, για τον προσδιορισμό του καταλληλότερου συμβατού δότη, μεταξύ λίγων προ-επιλεχθέντων υποψηφίων. Η Παγκόσμια Δεξαμενή BMDW διαθέτει στο μητρώο της 800.000 μονάδες ΟΠ.Α, παρ’ όλα αυτά όμως, στο πλαίσιο της HSCT, δεν είναι πάντοτε εφικτή η εύρεση δότη ΟΠ.Α με πλήρη HLA συμβατότητα με τον ασθενή-υποψήφιο λήπτη. Η εμφάνιση μετα-μεταμοσχευτικών επιπλοκών και GvHD παραμένουν σημαντικά εμπόδια στην πορεία της HSCT από ΟΠ.Α, καθώς, πολλοί ασθενείς αναγκάζονται να λάβουν HSC από έναν δότη ΟΠ.Α με τον οποίον έχουν ασύμβατα HLA αντιγόνα. Λόγω των πλεονεκτημάτων του ΟΠ.Α στην HSCT, έχει γίνει πλέον απαραίτητη η εύρεση εναλλακτικής στρατηγικής επιλογής του καταλληλότερου δότη για μεταμόσχευση μονάδας ΟΠ.Α, η οποία θα εστιάζει στην HLA συμβατότητα μεταξύ δότη και λήπτη. Μια στρατηγική αφορά στην επιλογή «αποδεκτών» HLA ασυμβατοτήτων που συνδέονται με χαμηλού βαθμού ανοσογονικότητα, όπως είναι τα ΝΙΜΑ του OΠ.Α. Σε ασθενείς με αιματολογικές κακοήθειες, που υποβλήθησαν σε μεταμόσχευση ΟΠ.Α, όπου υπήρχε συμβατότητα κατά τα ΝΙΜΑ, καταγράφηκε χαμηλότερο ποσοστό TRM και ταχύτερη εγκατάσταση του μοσχεύματος και, κατ’επέκταση, χαμηλότερο ποσοστό συνολικής θνησιμότητας, συγκριτικά με τις περιπτώσεις όπου δεν υπήρχε καθόλου HLA συμβατότητα μεταξύ του λήπτη και των ΝΙΜΑ της μονάδας ΟΠ.Α. Λαμβάνοντας υπόψιν τα ικανοποιητικά αποτελέσματα της μεταμόσχευσης ΟΠ.Α με συμβατότητα στα ΝΙΜΑ, προκύπτει το συμπέρασμα ότι τα ΝΙΜΑ θα μπορούσαν να λαμβάνονται υπόψιν κατά την επιλογή δότη ΟΠ.Α, με την προϋπόθεση ότι έχουν τυποποιηθεί τα HLA αντιγόνα της μητέρας του νεογνού-δότητου ΟΠ.Α. Κατά τη διαδικασία επιλογής του καταλληλότερου δότη ΟΠ.Α για μεταμόσχευση ορισμένων ασθενών, ενδεχομένως μία μονάδα ΟΠ.Α με συμβατά ΝΙΜΑ ως προς τον λήπτη να «υπερέχει» έναντι μιας μονάδας ΟΠ.Α με πλήρη HLA συμβατότητα. Προτείνεται λοιπόν μία εναλλακτική στρατηγική για τη διαδικασία αναζήτησης και εύρεσης HLA συμβατών μονάδων ΟΠ.Α, η οποία θα βασίζεται στον προσδιορισμό και τη χρήση των ΝΙΜΑ. Επίσης, εκτός της «βελτίωσης» της HLA συμβατότητας μεταξύ ενός υποψήφιου λήπτη HSC και μιας μονάδας ΟΠ.Α που φαίνεται να παρουσιάζει μία ή περισσότερες HLA ασυμβατότητες με τον λήπτη,τελικά η μεθοδολογία που βασίζεται στα ΝΙΜΑ θα συμβάλει και στον εντοπισμό δεύτερης, συνοδευτικής μονάδας ΟΠ.Α για έναν υποψήφιο λήπτη. Με τον τρόπο αυτό, θα μπορεί να χορηγηθεί μεγαλύτερη κυτταρική δόση στον λήπτη, στο πλαίσιο μιας διπλής μεταμόσχευσης ΟΠ.Α, βελτιώνοντας τις πιθανότητες για ταχύτερη εγκατάσταση του μοσχεύματος και μείωση των μετα-μεταμοσχευτικών επιπλοκών. Με τη χρήση της μεθοδολογίας που βασίζεται στα ΝΙΜΑ, η Ελ.Τ.ΟΠ.Α αποσκοπεί στην καθιέρωση μιας νέας πολιτικής στο πλαίσιο της αναζήτησης HLA συμβατών μονάδων ΟΠ.Α για ασθενείς, η οποία: α) θα συμβάλει στον γρήγορο εντοπισμό HLA συμβατών μονάδων ΟΠ.Α για ασθενείς με δυσκολία εύρεσης συμβατού δότη ΟΠ.Α και β) θα αυξήσει τον ρυθμό «απελευθέρωσης» μονάδων ΟΠ.Α από τις δεξαμενές κρυοκατάψυξης, περιορίζοντας τα προβλήματα αποθηκευτικού χώρου και επιπτώσεων στη βιωσιμότητα των κυττάρων, που προκαλούνται από τη χρόνια κρυοκατάψυξη των μονάδων ΟΠ.Α.
