Colonic carcinoma associated with an abnormal collagen table: Collagenous colitis

Abstract: A case of cecal adenocarcinoma with the typical histopathologic features of collagenous colitis throughout the resected colon is described in a 67‐year‐old woman. Collagenous colitis has not previously been reported in association with adenocarcinoma of the colon and the relationship of these findings appears to be unprecedented. A review of 100 randomly selected Duke's B adenocarcinomas of the colon revealed no similar case.

“…Perturbations in antioxidant status, oxidative stress and aberrant collagen production are associated with colon pathologies [1][2][3]14,15]. Oxidative stress induced by dietary vitamin E deficiency in this study was not reflected by major changes in the expression of the 207 genes screened using the stress gene array.…”

“…A particularly striking finding, to date, is the rarity of reported malignant disease complicating the clinical course of collagenous colitis. Colorectal cancer has been noted [10,29] , including cecal cancer [30] , but these may have been only coincidental. In an extensive survey study, the overall risk of colorectal cancer was similar to the general control population; only 2 patients with collagenous colitis were seen with colorectal cancer, but these occurred before development of colitis [31] .…”

“…Usually the change is apparent in a rectal biopsy, although not always. 22 The course of the illness is variable: some patients have chronic persistent diarrhoea'4; some have relapses and remissions"'3; and a few have apparent spontaneous resolution either with7' or without'"2 disappearance of the abnormal collagen band. Conventional medical treatment is usually ineffective,'2 although there are reports of improvement with metronidazole,"29 mepacrine,' prednisolone,'0'3 sulphasalazine," 26 and loperamide" and also with the homoeopathic remedy Natrum Mur.…”
