Colon cancer with micropapillary carcinoma component: a clinopathologic study of 9 cases

Abstract: Recently, colon cancer with micropapillary pattern (MPP) has been identified. MPP is defined as tight tufts surrounded by cleft-like space and lacking true fibrovascular cores and showing reverse polarity. In this article, we studied nine cases of colon cancer with MPP. MPP usually accounted for a minor component in total tumour volume, which ranged from 3 to 40% with a mean percentage of 19.2%. Associated histological subtype showed moderately differentiated tubular adenocarcinoma in all cases. The reverse po… Show more

“…The current IHC results were consistent with those reported in the literature (19,20,22), demonstrating that, when identifying the MPC of CRC, the sensitivity and specificity of villin is higher compared with that of E-cadherin and EMA. E-cadherin is one of the immunomarkers of MPC (27,37,38), but the current study and previous studies showed that it often had no staining or only partial staining (35).…”

“…The present bioinformatics analysis demonstrated that EMA had the lowest sensitivity in COAD compared with villin and E-cadherin, as reported by previous studies (20,22,23,25). A previous study reported that villin detection using IHC is useful in the detection of reverse polarity of MPP (19).…”

“…We defined PDC showing reversed polarity with empty spaces around the clusters as PDC with micropapillary (MP) features (Figure B). Reversed polarity was certified by villin immunohistochemistry . When PDC with MP features occupied all the ×20 objective lens field, we designated the corresponding case as MP PDC‐positive.…”

“…Villin is associated with elimination of polarity that the epithelium exhibits, thus altering tissue architecture ( 40 ). Thus, in CRCs containing a micropapillary pattern, villin IHC has been effective for tracking the polarity in this type of CRC ( 41 ). The specificity of villin as a marker of intestinal origin is limited, similar to CDX2, as positivity may also be marked in adenocarcinomas with intestinal differentiation arising from a wide variety of organs including stomach, lung, and ovary as well as in malignancies of other sites such as the endocervix and liver (the latter more rarely, though) ( 42 ).…”
