For such a common procedure there are many methods described for circumcision. There is literature about methods such as gomko clamp, mogan clamp, shang ring, plastibell, dorsal slit, etc. We aimed to evaluate results and complications of circumcision with bipolar diathermic blade. M Ma at te er ri ia al l a an nd d M Me et th ho od ds s: : 702 pediatric patients younger than 16 years circumcised with bipolar diathermic blade between May 2012 and July 2013, the data from 623 patients who returned for check-ups were retrospectively evaluated. Circumcision performed under local and/or sedation anesthesia. At the end of the circumcision, no patient was given any dressing and antibiotic treatment. Patients were called for check-up after the second day. Patients with full skin bonding between two sutures were accepted as having wound fully healed. The circumcision healing time and complication rates were evaluated. Cost analysis was completed. R Re es su ul lt ts s: : The average age of 623 patients was 6.7±5,3 years (6 days-16 years). The average operation duration and healing time of patients was 5.45±1,16 minutes and 4.3±0,8 days, respectively. Slight edema lasting 2-3 days was observed in 153 (24%) patients defined as minor complication. There was no major complication such as hemorrhage, burning and infection. The cost analysis found that each circumcision with bipolar diathermic blade had an average saving of 5.6 dollars. Purpose: the study was aimed at analyzing the complications that occur during circumcision and estimate the effectiveness of the developed original method of this surgery.Materials and methods. Over 6,000 observed results of circumcision performed in boys from 3 regions of the country using both conventional, and original author techniques were analyzed. They included saving resection of the prepuce preserving the inner leaf of the prepuce without sutures.Results. The first group included 3,223 boys who had a surgery following the technique made at the clinic when 0.3% of unsatisfied results were obtained. The second group consisted of 3,115 children who had a conventional surgery at medical institutions and at home with unsatisfactory results diagnosed in 7.6% of patients.Conclusions. The main reasons for complications in circumcision are the lack of a proper preoperative preparation of the surgical field, deviation from the conventional surgery technique and inad equate postoperative care. The sutureless circumcision technique developed by the author enabled to achieve minimum of complications.

Background Circumcision is performed as a routine operation in many countries, more commonly for religious and cultural reasons than for indicated conditions, such as phimosis and balanitis. There are many techniques available, and recently electrocautery and both Nd:YAG and CO2 lasers, instead of blades, have been used for skin and mucosal incisions. However, the infection risk in circumcisions performed using a CO2 laser was 10% higher. There are also reports of sutureless procedures using cyanoacrylate, but these have higher risks of hematoma and hemorrhage. We combined a CO2 laser and cyanoacrylate to shorten the operation time and to decrease bleeding complications.Materials and Methods : Circumcisions were performed under general anesthesia with CO2 laser and cyanoacrylate combination in 75 6–9-year-old boys between May 2013 and August 2014 only for religious reasons. As a control, we compared them retrospectively with 75 age-matched patients who were circumcised using the conventional guillotine method in our clinic.Results No hematomas, bleeding, or wound infections were observed. One wound dehiscence (1.33%) occurred during the early postoperative period and healed without any additional procedures. The median operating time was 7 (range 6–9) minutes. The conventional guillotine group comprised one hematoma (1.3%), two wound dehiscences (2.6%), and two hemorrhages (2.6%), and the median operating time was 22 (range 20–26) minutes. The difference in surgical time was significant (p<0.001), with no significant difference in the rate of complications between the two groups.Conclusion The combined CO2 laser and cyanoacrylate procedure not only decreased the operating time markedly, but also eliminated the disadvantages associated with each individual procedure alone.
