Collagenous Colitis.

Abstract: Collagenous colitis is characterized clinically by chronic watery diarrhea and pathologically by colonic mucosal subepithelial collagen deposition. Wereport a 72-year-old womanwho had collagenous colitis associated with chronic watery diarrhea. She received a non-steroidal antiinflammatory agent (sulindac) because of rheumatoid arthritis. Histological examination of biopsy showed a thick subepithelial collagen layer with lymphocytes, plasma cells, and infiltration ofa few eosinocytes in the lamina propria. The… Show more

“…Castanet et al [11] report that the course of the arthritis is less severe and not destructive. Collagenous colitis and arthritis may develop after treatment with parenteral gold and non-steroidal anti-inflammatory drugs (NSAIDs) [3,8,10]. However, our patient had not received gold or NSAID therapy at any time.…”

“…Because both conditions are recognised clinical entities with a certain prevalence (collagenous colitis being a rare disease), one should expect overlap of the two diseases in one patient with a statistically predictable prevalence. To our knowledge only four such cases have been reported in recent years [7][8][9][10]. Castanet et al [11] report that the course of the arthritis is less severe and not destructive.…”

“…16 These conditions need to be considered when selecting a population for study. Also collagenous colitis has been linked to the use of certain drugs including non-steroidal anti-inflammatory drugs, [17][18][19][20][21][22] simvastatin, lansoprazole, 23,24 and ticlopidine 25 and these may not have been documented in the case histories. Finally, the relapsing-remitting nature of collagenous colitis also makes interpretation difficult.…”

“…Clinically, it is characterized by watery, chronic, and nonbloody diarrhea. Its diagnosis, however, remains histological and the microscopic examination of mucosal biopsies reveals characteristic histopathological changes: an abnormal thickening of the subepithelial collagen layer (>10  μ m), as well as a lymphocytic infiltration of the epithelium and the lamina propria [4]. The etiology of CC remains enigmatic and is multifactorial with different elements being more influential in different individuals.…”
