BackgroundThe U.S. has experienced exponential growth in overdose fatalities over the past four decades and more than 22 million people are currently living with a substance use disorder (SUD). While great strides have been made in advancing the science of SUD prevention and treatment, proven programs and interventions are not commonly disseminated at scale in impacted communities. The U.S. Cooperative Extension System (Extension) has been recognized as a valued partner in addressing SUD in communities. Federal funding supporting Extension's response to the opioid epidemic reached $35 million in 2021 primarily through two grant programs: the United States Department of Agriculture's (USDA) Rural Health and Safety Education program; and the Substance Abuse and Mental Health Services Administration (SAMHSA) Rural Opioid Technical Assistance (ROTA) grants. The primary objective of this scoping review was to identify the range of Extension activities aimed at mediating substance misuse.MethodsAuthors utilized the PRISMA-SCR model to complete this scoping review. Due to the nature of Extension work and the expectation that few activities would be cited in the peer-reviewed literature, the scoping review included a search of peer-reviewed databases, Extension websites for each state and U.S. territory, and the utilization of a web search engine. Upon initial analysis of records returned, authors noted a discrepancy between results returned and the number of states receiving ROTA grants. Thus, authors supplemented the PRISMA-SCR review protocol with a systematic procedure for investigating ROTA funded activities not readily apparent in the peer-reviewed or grey literature.ResultsA total of 87 records met inclusion criteria. Findings included seven peer-reviewed articles and 80 results from the grey literature. An additional 11 ROTA grantees responded to requests for information regarding state level activities.ConclusionsNationwide, Extension has scaled multiple efforts to address SUD operating through a loose confederation of organizations connected to the land-grant system. Most activities are funded by federal grants and focus on state-sponsored training and resource sharing. The volume of effort is significant, however, implementation at the community-level has been slow. Significant opportunities exist for local adoption of evidence-based practices aimed at mitigating SUD.

Since its beginning, the opioid crisis has been unrelenting and remains one of the most challenging public health problems to solve. This is especially true in rural communities which are disproportionately impacted. Although some initiatives to address overdose and poisoning deaths have found success, significant challenges remain (e.g., fentanyl, workforce shortages). To close this gap, it will take shared leadership and partnership with community-based organizations as key collaborators to strengthen health systems and related opioid outcomes. The present article proposes an expanded role for the nation's Cooperative Extension System in opioid prevention, treatment, and recovery through a local Health Extension Agent. Leveraging the extension system is especially important in rural and underserved communities. Building on key strengths of the extension network and guidance of national frameworks, this role would serve as an intermediary linking community members and programs, health care providers, and addiction prevention and treatment experts to facilitate access and uptake of evidence-based promotion, prevention, treatment, and recovery strategies. We propose core functions of a Health Extension Agent including (a) provision of training and technical assistance, (b) facilitation of community partnerships and shared resources, and (c) dissemination of evidence-based programs and/or approaches driven by community need. Given the key strengths of extension, this system is well poised as a key contributor to addressing the behavioral health continuum from promotion through recovery related to opioid misuse. Public Health Significance StatementRural communities are disproportionately impacted by opioid use and related consequences. Collaboration between health providers and community organizations is critical to a comprehensive approach addressing opioid use. This article describes how a Health Extension Agent approach can facilitate collaboration to support rural behavioral health.

Compassion Fatigue, Resilience, and Endurance of Special Educators in Rural P–12 and Higher Education

The effects of mental health challenges in the United States are growing and affect both P-12 special education teachers and faculty at institutions of higher education (IHE). Educators struggle with compassion fatigue and associated burnout. With educators leaving the field for a plethora of reasons, it is vital to examine factors that support those who remain in education. Thus, we analyzed factors that support the retention of educators at both levels of instruction. In a qualitative analysis of data from survey respondents ( N = 200), we examined the resiliency and endurance of practicing rural P-12 special educators and faculty at IHEs. Results from inductive content analysis indicate that factors leading to persistence in education begin with educators finding meaning in their teaching. Educators who demonstrate career endurance have a strong sense of self-efficacy in their teaching abilities, receive emotional benefits from their students, and have quality opportunities to mentor and collaborate with others. By contrast, some educators indicate burnout is a key factor for leaving the field, and reported reasons include low pay, paperwork, poor work/life balance, lack of respect, and the mental health of themselves and their students. Based on these findings, we offer recommendations for research and practice.
