Colistin and tigecycline resistance in carbapenemase-producing Gram-negative bacteria: emerging resistance mechanisms and detection methods

Abstract: SummaryA literature review was undertaken to ascertain the molecular basis for tigecycline and colistin resistance mechanisms and the experimental basis for the detection and delineation of this resistance particularly in carbapenemaseproducing Gram-negative bacteria. Pubmed, Google Scholar and Science Direct were searched with the keywords colistin, tigecycline, resistance mechanisms and detection methods. Trans-complementation and comparative MIC studies, mass spectrometry, chromatography, spectrofluorometry… Show more

“…Antimicrobial resistance (AMR) is inarguably one of the greatest threats and challenges to clinical medicine and public health in this century (Laxminarayan et al., ). Antimicrobial‐resistant microbes, particularly bacteria and fungi, are increasingly being reported in healthcare and community settings, with high attendant morbidities, mortalities, and healthcare‐associated costs that runs into millions of dollars (Laxminarayan et al., ; Osei Sekyere, ; Osei Sekyere, Govinden, Bester, & Essack, ). Until recently, AMR was mainly reported in bacteria.…”

“…Genotypic tests, like automated real-time multiplex polymerase chain reaction (PCR) and microarray platforms, are examples of emerging diagnostic methods [19, 20]. Curetis Unyvero (Curetis AG) is a multiplex PCR hybridization system designed to identify pneumonia pathogens and resistance genes in respiratory samples [21].…”

“…The combined emergence of plasmid‐mediated carbapenem and colistin resistance, has thrown clinical medicine into a state of shock and therapeutic conundrum as infection control and management have become more difficult with little or no therapeutic options (Osei Sekyere, ; Osei Sekyere, & Asante, ; Osei Sekyere, Govinden, Bester, & Essack, ). Prior to the advent of carbapenem and colistin resistance in Gram‐negative bacteria, these two antibiotics were, respectively, used as last‐resort antibiotics for fatal and multidrug‐resistant infections (MDRIs) mediated by extended‐spectrum β‐lactamases (ESBLs) and carbapenemases (Osei Sekyere et al., ). Colistin and tigecycline alone, or in combination with an aminoglycoside, a fluoroquinolone, a carbapenem or fosfomycin, were used in managing carbapenem‐resistant infections before the emergence of colistin resistance (Osei Sekyere et al., ).…”
