BACKGROUND: A cold chain is a temperature-controlled supply chain with uninterrupted chain of activities that maintain a given temperature range that keep and maintain the quality of vaccines. Vaccines move through complex procedures and processes that require special attention and care. Effective cold chain systems require efficient end-to-end vaccine storage, handling, and stock management to maintain vaccines under strict temperature control of between 2 °C and 8 °C (for almost all vaccines). METHODS: A descriptive cross-sectional study design and mixed (qualitative and quantitative) research approach is employed to conducting the research. Data were collected through face to face in-depth interviews and questionnaires from Vaccinators and key persons from IPs. Prior to data collection, ethical approval was obtained from national Ministry of Health, Directorate of planning and M&E, and the University of Rwanda, Research Committee Board. Accordingly, data were collected after seeking the personal consent sought from the participants. RESULTS: The key findings from this study showed that the cold chain coverage has been improved compared to the result obtained in EPI coverage survey conducted in 2017. The results revealed that inadequate availability of EPI cold technicians, lack of fridge spare parts, trained staff, and adequate vaccine forecasting was the major challenges at county and the health facility level. CONCLUSIONS: The main factor that contributed positively in strengthening vaccines supply chain system in the Upper Nile State was the substantive increase in cold chain coverage. However, the role of the CCEOP in improving immunization coverage is still doubted due to lack of regular preventive maintenance, spare parts, and EPI technicians. The major challenges that affect the vaccines relevance, effectiveness, efficiency and sustainability were found to be mainly poor vaccines handling, due to inadequate knowledge on cold chain management.

Background Vaccines require cold chain storage conditions, and good distribution practices throughout the supply chain to maintain their quality and potency. However, in the last mile of the vaccines supply chain, these requirements may not be guaranteed resulting in reduced effectiveness which could lead to an upsurge in vaccine preventable morbidity and mortality. The aim of this research was to evaluate vaccine storage and distribution practices in the last mile of vaccine supply chain in Turkana County. Methods A descriptive cross-sectional study was conducted from January 2022 to February 2022 across seven sub-counties in Turkana County, Kenya, to assess vaccine storage and distribution practices. The study sample size was 128 county health professionals across 4 hospitals, 9 health centers, and 115 dispensaries. The respondents were selected using simple random sampling within the facilities strata. Data were collected using a structured questionnaire, adapted, and adopted from a standardized WHO questionnaire on effective vaccines management and administered to one healthcare personnel working in the immunization supply chain per facility. Data were analyzed using excel and presented as percentages in table forms. Results A total of 122 health care workers participated in the study. Most respondents (89%, n = 109) had utilized a vaccine forecasting sheet, but only 81% did have an established maximum–minimum level inventory control system. Many of the respondents had sufficient knowledge of ice pack conditioning although 72% had adequate vaccine carriers and ice packs. Only 67% of respondents had a complete set of twice-daily manual temperature records at the facility. Most refrigerators complied with the WHO specifications but only 80% of them had functional fridge-tags. The number of facilities that had a routine maintenance plan was below average while only 65% had an adequate contingency plan. Conclusion Rural health facilities have suboptimal supply of vaccine carriers and icepacks for effective storage and distribution of vaccines. In addition, some vaccine fridges lack functional fridge-tags for proper temperature monitoring. Routine maintenance and contingency plans remain a challenge to ensure optimal service delivery.

Menurut Badan Pengawas Obat dan Makanan No.9 tahun 2019 dalam setiap aspek dan rangkaian distribusi obat salah satunya pada produk rantai dingin. Produk rantai dingin merupakan obat yang labil, sensitif terhadap suhu perlu disimpan dalam lemari pendingin dengan suhu antara 2-8 C. Tujuan penelitian mengevalusi bagaimana operasional, personil/pelatihan dan bangunan/fasilitas, terhadap penerapan distribusi produk rantai dingin pada empat puskesmas di Kabupaten Karawang. Penelitian ini bersifat observasional yang berbentuk deskriptif dengan pendekatan metode kualitatif. Pengambilan sampel dilakukan dengan menggunakan teknik purposive sampling. Semua kegiatan Alur distribusi pada puskesmas tersebut memiliki alur distribusi yang sama. Petugas penanggung jawab merupakan tenaga kesehatan yang telah mengikuti program pelatihan imunisasi, dari empat puskesmas 50 % masih belum memiliki bangunan dan ketidaksesuaian jarak/kondisi penyimapan dengan ketetapan standar. Kelengkapan alat penunjang dalam pengelolaan produk rantai dingin semua peralatan mematuhi seperangkat standar kinerja. Jadi, dari ke empat puskesmas yang berada di kabupaten karawang dengan presentase hasil nilai skala guttman yaitu 83,3 % efektif dalam pendistribusian, personil/petugas, pengelolaan produk rantai dingin vaksin dan juga setiap puskesmas dilengkapi oleh standar operasional prosedur. Pada puskemas untuk meningkatkan implementasi CDOB khususnya pengelolaan produk rantai dingin salah satunya pada aspek bangunan dan praktisi penanggung jawab.
