Sie können erworben oder vererbt werden und fokal oder systemisch lokalisiert auftreten. Dabei unterscheidet man mehrere Gruppen: AA-Amyloidose, die meist reaktiv, sekundär auf dem Boden chronischer Entzündungen, Erkrankungen oder Infektionen auftritt. Dabei ist die Niere das am häufigsten und der Gastrointestinaltrakt das am zweithäufigsten befallene Organ (12). AL-Amyloidose mit häufigstem Vorkommen bei monoklonaler Gammopathie und 15% der Myelompatienten; die seltene familiäre polyneuropathische Amyloidose (Punktmutation, autosomal-dominant); die β2Mg-Amyloidose, die dialyseassoziiert ist und sich am häufigsten über gastrointestinale Blutungen im Magen-Darm-Trakt manifestiert; und zuletzt die senile Amyloidose (36% aller autopsierten Patienten, die älter als 85 Jahre wurden), die nicht bedeutsam ist.Bisher sind in der Literatur nur wenige Patienten mit AAAmyloidose bei bestehender chronisch entzündlicher Darmerkrankung beschrieben worden. Nur bei knapp 20% der Fälle wurde über eine speziell zur Behandlung der Amyloidose eingeleitete Therapie berichtet. Die Prognose der Betroffenen war trotz medikamentöser oder operativer Therapie besser, je früher eine Therapie eingeleitet wurde.Die Angaben über die Häufigkeit der Koinzidenz der beiden Erkrankungen in Europa variieren von 0,5 -6%, wobei die höhe-ren Zahlen aus Untersuchungen von histologischen Präparaten von Autopsien und Darmresektaten (17, 27) stammen. Eine prospektive Studie zur Feststellung der Inzidenz (30) wurde nach 177 Patienten ohne einen positiven Befund abgebrochen.Die Prognose der beschriebenen Fälle wird insgesamt als sehr ungünstig angegeben und ist vor allem ohne Therapie (44, 45, 48) tödlich. Nach Diagnosestellung betrug die mediane Überlebenszeit früher etwa 2 Jahre (13, 16, 17,44), ohne Therapie sogar nur 0,9 Jahre und war vor allem vom Nierenbefall, der mit > 90% (12) am häufigsten auftritt, abhängig. In neueren Arbeiten (29, 45) wird die Überlebenszeit mit 4-5 Jahren höher angegeben. Das ist sicherlich auf die besseren konservativen Therapiemöglichkeiten einer terminalen Niereninsuffizienz mit Dialyse und Nierentransplantation zurückzuführen (29, 45). Ein frühzeitiger Therapiebeginn wird von fast allen Autoren als prognostisch vorteilhaft angesehen (13, 17,44,45,48) und kommt vor allem Patienten mit nur geringer Nierenfunktionseinschränkung zugute. Durch eine medikamentöse Therapie mit Immunsuppressiva und Colchizin wurde in fast allen in der Literatur beschriebenen Fällen eine Remission erzielt.
