Purpose We provide detailed instructions for analyzing patient-reported outcome (PRO) data collected with an existing (legacy) instrument so that scores can be calibrated to the PRO Measurement Information System (PROMIS) metric. This calibration facilitates migration to computerized adaptive test (CAT) PROMIS data collection, while facilitating research using historical legacy data alongside new PROMIS data. Methods A cross-sectional convenience sample (n = 2,178) from the Universities of Washington and Alabama at Birmingham HIV clinics completed the PROMIS short form and Patient Health Questionnaire (PHQ-9) depression symptom measures between August 2008 and December 2009. We calibrated the tests using item response theory. We compared measurement precision of the PHQ-9, the PROMIS short form, and simulated PROMIS CAT. Results Dimensionality analyses confirmed the PHQ-9 could be calibrated to the PROMIS metric. We provide code used to score the PHQ-9 on the PROMIS metric. The mean standard errors of measurement were 0.49 for the PHQ-9, 0.35 for the PROMIS short form, and 0.37, 0.28, and 0.27 for 3-, 8-, and 9-item-simulated CATs. Conclusions The strategy described here facilitated migration from a fixed-format legacy scale to PROMIS CAT administration and may be useful in other settings.

Predictors of study retention and scheduled visit attendance in the University of North Carolina Center for AIDS Research (UNC CFAR) prospective clinical cohort of HIV-infected patients enrolled between 1 January 2001 and 1 January 2008 are reported. At study entry, 1636 participants were 32% female, 58% were African-American, 49% had not received HIV care elsewhere, 71% were receiving or initiated combination antiretroviral therapy, and 26% were diagnosed with AIDS, with median (quartiles) age of 40 (34; 47) years, distance to clinic of 45 (21; 70) miles, HIV-1 RNA of 1396 (200; 26,750) copies/ml, and CD4 of 374 (182; 602) cells/mm 3 . Participants contributed a median of 7 (4; 13) scheduled visits and 2.25 (1.0; 3.9) years alive under follow-up. During 6134 person-years of follow-up, 414 participants dropped out and 145 died. Accounting for differences in death by participant characteristics, the 6-year cumulative probability of retention was 67% [95% confidence limits (CL): 65, 70%], with 6.75 (95% CL: 6.13, 7.43) drop outs per 100 person-years. In a multivariable Cox proportional hazards model, retention was higher among participants who were insured, had not received HIV care elsewhere, had controlled HIV viremia, and were living in nonurban areas or proximate to the clinic. In a multivariable modified Poisson regression model that accounted for differences in drop out and death by participant characteristics, visit attendance was higher among older, AIDS-diagnosed, immune compromised, and cARTinitiated participants. The UNC CFAR clinical cohort has ample enrollment with retention and visit attendance modestly influenced by factors such as disease severity.

Prevalence and Predictors of Substance Use Disorders Among HIV Care Enrollees in the United States

Prior efforts to estimate U.S. prevalence of substance use disorders (SUDs) in HIV care have been undermined by caveats common to single-site trials. The current work reports on a cohort of 10,652 HIV-positive adults linked to care at seven sites, with available patient data including geography, demography, and risk factor indices, and with substance-specific SUDs identified via self-report instruments with validated diagnostic thresholds. Generalized estimating equations also tested patient indices as SUD predictors. Findings were: 1) a 48% SUD prevalence rate (between-site range of 21–71%), with 20% of the sample evidencing polysubstance use disorder; 2) substance-specific SUD rates of 31% for marijuana, 19% alcohol, 13% methamphetamine, 11% cocaine, and 4% opiate; and 3) emergence of younger age and male gender as robust SUD predictors. Findings suggest high rates at which SUDs occur among patients at these urban HIV care sites, detail substance-specific SUD rates, and identify at-risk patient subgroups.
