Cognitive profile in idiopathic autonomic failure: relation with white matter hyperintensities and neurofilament levels

Abstract: Objective: To disclose the nature of cognitive deficits in a cohort of patients with idiopathic autonomic failure (IAF) by exploring the relation among cognitive functions, cardiovascular autonomic failure (AF) and clinical progression to another a-synucleinopathy (phenoconversion). Methods: We retrospectively identified all patients with a clinical diagnosis of IAF who underwent a comprehensive neuropsychological evaluation, clinical examination and cardiovascular autonomic tests from the IAF-BO cohort. Brain… Show more

“…Chen et al was the only study investigating non-hypertensive patients [ 44 ]. Cani et al examined the incidence of supine hypertension in a cohort of patients with idiopathic autonomic failure (IAF) [ 66 ]. Eight studies did not comment on hypertension history status of their population cohort [ 25 , 27 , 46 , 51 , 52 , 60 , 62 , 67 ] while five [ 25 , 27 , 51 , 52 , 67 ] of them stated that some patients received antihypertensive treatment.…”

“…6 While this study did not assess cognitive endpoints, others have established a strong association between higher WMH burden and cognitive decline in synucleinopathies. 7 Of importance, treatment with antihypertensive drugs reduces the risk of all-cause dementia and cognitive impairment. 8 Therefore, hypertension must be taken into consideration in works investigating risk factors of cognitive decline.…”
