Cognitive-behavioral Case Formulation

Abstract: This chapter begins with a description of the hypothesis-testing approach to clinical work of which the case formulation is a part. We discuss the role of the case formulation in treatment and review evidence that the formulation contributes to the effectiveness of treatment. We also describe the elements of the cognitive-behavioral (CB) case formulation and present a case to illustrate the process of developing a formulation and using it to guide treatment.

“…Formulation is central to helping clients understand the presenting problems, hypothesizing potential causes and maintaining factors, and creating a cohesive picture of the client's experience to guide treatment selection. 27 Although there was consensus that formulation was important, qualitative comments revealed that clinicians found communicating formulation particularly challenging when using telephone instead of videoconferencing. As such, guidelines on adapting formulation to the telehealth context might be particularly useful to clinicians.…”

“…One purpose of a formulation is to tie together the elements of a case (origins, mechanisms, precipitants, problems) into a coherent narrative so they can be understood as a whole rather than as a list of disparate unrelated facts. The case formulation is presented in a paragraph, as in the example above, or in a diagram with arrows (for examples, see O'Brien & Haynes, 1995;Persons & Davidson, 2001).…”

“…In this chapter we make several substantial changes to our earlier presentations (Persons, 1989;Persons & Davidson, 2001;Persons, Tompkins, & Davidson, 2000), including those in the previous edition of this volume (Persons & Tompkins, 1997). We present the formulation as one element of a hypothesis-testing approach to clinical work; we rely more on diagnosis, we allow for formulations based on conditioning and emotion theories (in our previous work, formulations were always based on Beck's cognitive theory), we simplify the format of the formulation, and we describe a worksheet that aids in the process of developing a case formulation.…”

“…Coping or compensatory strategies (such as social isolation or vigilance) can be helpful in dealing with symptoms, or at times they can become problems themselves (Leahy et al, 2005). The case conceptualization serves to illustrate the origins of the problems and a working hypothesis of how the various factors and reactions above might be linked sequentially, suggesting a plan of treatment and a prediction of what obstacles to treatment might arise and how to overcome them (Persons & Tompkins, 1997).…”
