Psoriasis vulgaris and lichen planus are distinct T‐cell‐driven inflammatory skin diseases. Both present in a variety of clinical subtypes. Mucosal or nail involvement may be present. Here, we report the rare concomitant clinical presentation of psoriatic arthritis and annular atrophic lichen planus on the trunk of a 52‐year‐old male patient. Treatment with sulfasalazine failed to control inflammatory activity; methotrexate and leflunomide were ceased due to side‐effects. After confirmation of both diagnoses, we initiated a tumor necrosis factor (TNF)‐α‐directed therapy with the fusion protein etanercept resulting in significant improvement of both conditions. This case report aims to highlight the rare colocalization of psoriasis and lichen planus, the rare entity of annular atrophic lichen planus, and to discuss a possible beneficial impact of certain TNF‐α inhibitors on subtypes of lichen planus.

Przegląd Dermatologiczny 2016/2 StreSzczenieWprowadzenie. Łuszczyca i liszaj płaski należą do powszechnych zapalnych schorzeń dermatologicznych, których współwystępowanie jest rzadko opisywane. Etiopatogeneza obu chorób nie jest do końca poznana, ale czynniki autoimmunologiczne odgrywają istotną rolę w ich rozwoju. Wspólną cechą jest także występowanie objawu Köbnera. Zasadnicze różnice dotyczą obrazu klinicznego, histopatologicznego oraz zjawisk immunologicznych. Cel pracy. Przedstawienie przypadku współistnienia łuszczycy zwyczajnej i liszaja płaskiego błon śluzowych. Opis przypadku. Pacjentka 75-letnia z 3-letnim wywiadem liszaja pła-skiego błon śluzowych zgłosiła się do Kliniki z powodu utrzymujących się od 2 lat zmian rumieniowych zlokalizowanych w okolicach wyprzeniowych. Leczenie ambulatoryjne miejscowymi i ogólnymi preparatami przeciwgrzybiczymi nie spowodowało poprawy stanu miejscowego. Na podstawie badania histopatologicznego wycinka pobranego ze zmiany skórnej rozpoznano łuszczycę. W leczeniu zastosowano miejscowe preparaty dziegciowe oraz pochodne kalcyneuryny z dobrym efektem. Wnioski. Współwystępowanie łuszczycy i liszaja płaskiego jest zjawiskiem rzadkim. Przegląd dostępnego piśmiennictwa wskazuje na istnienie możliwych, jednak w dalszym ciągu niewyjaśnionych powiązań między tymi dermatozami. AbStrActIntroduction. Psoriasis and lichen planus are common dermatological conditions, whose coexistence is rarely described. Although the exact etiopathogenesis of both diseases is still unknown, autoimmune factors play an important role in their development. A common feature is also the occurrence of the Köbner phenomenon. The principal differences concern the clinical, histological and immunological events. Objective. To present a patient with oral lichen planus, in whom psoriasis developed. Case report. A 75-year-old patient, with a 3-year history of oral lichen planus, presented itchy erythematous skin lesions initially located in the intertriginous area. Outpatient treatment consisting of topical and oral antifungal medications did not bring significant improvement. Skin biopsy confirmed the diagnosis of psoriasis.
