Purpose To describe a case of a medial-type persistent trigeminal artery (PTA) associated with multiple arterial variations. Methods A 34-year-old woman with multiple sclerosis underwent cranial magnetic resonance (MR) angiography from the aortic arch to the neck region and intracranial region for the evaluation of an unruptured cerebral aneurysm that was previously detected on MR imaging. The MR machine was a 3-T scanner. Results There was an aberrant right subclavian artery and bicarotid trunk, medial-type left PTA and ipsilateral posterior communicating artery (PCoA) supplying bilateral posterior cerebral arteries (PCAs). The unruptured aneurysm was located at the paraclinoid segment of the left internal carotid artery and was treated successfully by coil embolization via a transfemoral approach. Conclusion Only 10% of PTAs are classified as the medial type. The association with hyperplastic ipsilateral PCoA, which supplies the bilateral PCAs, has not been previously reported. Aberrant right subclavian arteries are common and are frequently associated with a bicarotid trunk. Before catheterization of the cerebral arteries, the aortic arch and its branches must be evaluated by MR angiography or computed tomography angiography to prevent catheterization failure via the right transradial approach.

Artéria subclávia direita aberrante é a anomalia mais comum da aorta e comumente descrita como tendo sua origem após a origem da artéria subclávia esquerda. A dificuldade de deglutição é o sintoma mais comum quando a artéria aberrante causa compressão do esôfago, normalmente, pela presença de aneurisma em sua porção proximal. Estudos alertam para o risco de lesões iatrogênicas e dificuldades em concluir exames de imagem durante procedimentos pré e perioperatórios em pacientes com artéria subclávia direita aberrante. A escolha pelo melhor manejo cirúrgico ainda é debatida e a decisão de intervir deve ser individualizada. Pacientes assintomáticos podem não necessitar de nenhuma intervenção, entretanto, devem ser acompanhados para o monitoramento de possíveis alterações no diâmetro do vaso aberrante. O presente estudo relata o caso de uma paciente com artéria subclávia direita aberrante e discute aspectos anatômicos e clínicos dessa anomalia congênita.

Congenital pseudodiverticula of the esophagus are very rare. This case report describes the presentation, management and histopathology of a peudodiverticulum of the cervical esophagus in a neonate. The infant presented with respiratory distress and a right neck mass that required surgical excision. Pathology revealed a pseudodiverticulum that contained ectopic thymic, thyroid, and parathyroid tissue within the wall of the lesion. The presence of ectopic tissues of branchial origin and an aberrant right subclavian artery suggest an error in branchial development and neural crest cell migration.
