Abstract: The modified Leiden Convention is not a strict classification but a simple coronary coding system that is broadly applicable.

“…The usual three-leaflet semilunar valve configuration in mammals and birds consists of two sets: one in the aorta and one in the pulmonary trunk. 117 As a congenital malformation in humans, bicuspid valves are quite common (see also "Evo-devo aspects of congenital malformations"). Reptiles nearly always present with bicuspid semilunar valves in all three major arteries, two persisting aortas.…”

“…67 It is noteworthy that also in these events of cardiogenesis, asymmetric development is apparent, although the main leaflets of the aortic and pulmonary semilunar valves originate from the same two OFT endocardial cushions. Only the so-called nonfacing leaflets 117,133 may have a different contribution of NCCs. Indeed, aortic bicuspidy in humans is far more common than pulmonary bicuspidy, as is also supported by studying a hamster strain.…”

“…To the best of our knowledge, furthermore, the association of transposition, unobstructed right aortic arch, and BAoV has yet to be reported. [1][2][3][4][5][6][7][8] Even if there is nothing in terms of development that would link together the lesions we have described, their unique association, together with the surgical implications, prompted us to report this observation.…”

“…[2][3][4][5] An aortic valve with two leaflets, the most frequent congenital heart defect, with an incidence of 0.5% to 2.0%, 6 is also very rare in the presence of transposition. 7,8 Its presence, nevertheless, can constitute reason for concerns in relationship to the transfer and reimplantation of the coronary arteries at the time of surgery. In the light of these findings, we report here the association of transposition, an unobstructed right aortic arch, and a bicuspid aortic valve (BAoV) in a neonate who underwent an arterial switch operation.…”

“…During the past decades, several nomenclatures have been presented to classify the coronary artery anatomy in patients with transposition of the great arteries (TGA). In this issue of the Journal, Gittenberger-de Groot and colleagues 1 have proposed a modified version of the widely used Leiden Convention. The purpose of the modification is to extend the use of Leiden Convention from the TGA setting into the setting of a bicuspid aortic valve and normally related great arteries.…”
