Cocooning of an iris-fixated intraocular lens in a 3-year-old child after perforating injury: Clinicopathologic correlation

Abstract: We present a 3-year-old patient who developed extensive noninflammatory fibrous membrane formation in the anterior chamber involving the present iris-fixated intraocular lens (IOL), with subsequent need for explantation. The child had had repair of a corneal injury, crystalline lens extraction and Artisan iris-claw IOL (Ophthec) implantation after a penetrating injury 2 months before. Histopathologic analyses were consistent with a retrocorneal membrane that originated from the corneal stroma and formed a "coc… Show more

“…Severe hyperopia developed in only one eye. However, postoperative refractive error was more than previous reports of either Artisan IOL 1,7,12 or iris fixated PCIOL. 13 One reason might be inaccurate keratometry in most of our cases due to irregular astigmatism caused by previous repaired corneoscleral laceration.…”

“…Except The Artisan IOL have been used safely for secondary implantation in adults and children with aphakia. 1,7,[9][10][11][12] They have also been used after traumatic injuries to the globe. [9][10][11][12] Gü ell et al 7 reported satisfactory results of Artisan IOL implantations in 16 aphakic patients.…”

“…2,7-9 They have also been used after traumatic injuries to the globe. 9,10 Van der Pol and Worst 11 describe the use of Artisan aphakic IOLs in children with congenital, developmental, and traumatic cataracts, and their results are similar to those in other reports concerning the use of posterior chamber IOLs. The surgical technique is relatively common in all the studies, placing the IOL horizontally so the haptics are enclavated on the nasal and temporal sides of the iris mid stroma.…”
