Cochlear implantation as a durable tinnitus treatment in single-sided deafness

Abstract: CI can successfully be used as treatment of severe tinnitus in patients with single-sided deafness and is equally effective for pure tone, narrow band noise, and polyphonic tinnitus. Long-term results show that implantation provides durable tinnitus relief in these patients. These results support the hypothesis that physiopathological mechanisms after peripheral deafferentation are reversible when hearing is restored. Single-sided deafness accompanied by severe tinnitus is a new indication for CI.

“…First outcome measures with respect to speech intelligibility in single-sided deaf patients have confirmed that performance significantly improved with a CI compared to monaural acoustic hearing before implantation and compared to other hearing aid systems [Arndt et al, 2011a;Firszt et al, 2012a, b]. Additional benefits after CI switch-on were better sound localization [Kamal et al, 2012;Mertens et al, 2016a;Vlastarakos et al, 2013], the reduction of ipsilateral tinnitus [Arts et al, 2012;Kleine Punte et al, 2013;Mertens et al, 2016a;Punte et al, 2011;Song et al, 2013] and the improvement of quality of life [Härkönen et al, 2015].…”

“…The tinnitus relief resulting from CI stimulation was probably an important motivating/driving factor for constant/daily CI use [Van de Heyning et al, 2008;Punte et al, 2011]. This reinforcement to wear a CI may have an influence on the speech reception results obtained with a CI in SSD patients.…”

“…In SSD adults, several studies demonstrated improvements in qualities supporting higher-level processing and integration of both acoustic and electrical signals leading to improved overall hearing. The improvements in performance reported for younger CI patients with acquired SSD suggest that results were equivalent to those seen for SSD adults and show evidence of rehabilitation of binaural hearing and similar improvement in the quality of speech comprehension in noise and the ability for sound localisation [Arndt et al, 2011a, b;Punte et al, 2011;Stelzig et al, 2011;Firszt et al, 2012;Plontke et al, 2013].…”
