Cochlear implantation and hearing preservation: Results in 21 consecutively operated patients using the round window approach

Abstract: There was no incidence of total loss of residual hearing in any of the patients. A slight deterioration of low frequency thresholds occurred in some patients. Mean hearing loss at 125-500 Hz was 14.4 dB at 1 month following surgery and 15.6 dB after 1 year. Insertion angle (300-540°) and depth (17.5-28.5 mm) were not statistically correlated to hearing loss.

“…The round window is used as an insertion site for cochlear implant electrodes and has also recently been used as an implantation site for hearing aid transducers (41)(42)(43). Clear visualization of the round window using a SWIR otoscope could provide an alternative to radiographic imaging for evaluation of such surgical implants.…”

“…The distance between the RW and OW may foresee the 'hook' size. In our view, a transmembrane RW approach to introduce the CI electrode array may be safer to avoid damage to these structures, particularly in patients with partial deafness or substantial residual hearing [Erixon et al, 2012].…”

“…Patient selection for CI surgery has changed in recent years since residual hearing can now be preserved in a high number of cases Bruce et al, Erixon et al, 2012]. Patients with partial deafness having normal or near-normal low-frequency hearing can nowadays be operated and use combined electric acoustic stimulation.…”

“…Woodson et al [2010] used the Hybrid-S electrode in 81 patients and achieved a mean increase in low-frequency threshold of 14.6 dB after 1 month. In another study by Erixon et al [2012], the MedEl Flex implant (24 mm) was used in 21 patients with a median hearing loss of 14.4 dB reported after 1 month, and in 11 patients with a median hearing loss of 15.6 dB after 1 year.…”
