Cobb's collar: a rare cause of urinary retention

Abstract: Cobb's collar is a membranous stricture of the bulbar urethra, often misdiagnosed, potentially leading to acute urinary retention, upper urinary tract dilations, enuresis, infectious diseases, poor streaming and haematuria. A 14-year-old boy with a history of previous hospitalisations came to our attention. A Cobb's collar was diagnosed and successfully managed with a videoendoscopic one-step approach. Although extremely uncommon, paediatric urologists must be aware of this congenital anomaly. A correct and ti… Show more

“…5 Intervention has shown improvement in bladder irritability, VUR, urethral calibre and uroflowmetry as well as a reduced presentation with UTI and enuresis. 5,7 This is the first reported case of a Cobb's collar in association with VACTERL. VACTERL was first described in 1973 and is defined by the presence of at three of the described abnormalities, with anorectal malformations and trachea-oesophageal fistulae considered core components.…”

“…Chronic urinary retention and bilateral hydronephrosis may be a presenting feature in some patients, as in our case. [ 10 11 ] These adult patients may also complain of poor ejaculation. [ 5 ] Late presentation of Cobb's collar (caused by a persistence of urogenital membrane) can be differentiated from acquired stricture by the absence of the characteristic folds between COPUM anomalies and verumontanum.…”

“…There is no information related to male cases in the literature, however, for differential diagnosis, anterior urethral pathologies are important. When anterior urethral pathologies are examined, congenital urethral polyp in the anterior urethra, fossa navicularis pathologies (stenosis, Guerin's sinus), anterior urethral valve, syringocele (dilatation of Cowper's gland ducts), congenital bulbous urethral stricture, condyloma accuminatum, and especially botryoid sarcoma polyps should be taken into the consideration in the differential diagnosis (5,6,7,8). Definite diagnosis of squamous cell papilloma is made by careful pathological investigation.…”
